外科手消毒规范化培训长效机制的研究

Strategies to build long-term mechanism of standardized training for the department of surgery to realize hand disinfection

目的探讨构建手术室外科手消毒规范化培训的方法和临床价值,为手术室手消毒环节控制医院感染率提供参考依据。方法分别选取开展手术室外科手消毒规范化培训前后医护人员各68名,分别为观察组与对照组,使其在各方面均有可比性;通过细化指标对两组医护人员手消毒执行情况进行评价;于手术不同时间采样进行手细菌培养,对消毒效果进行评价,采用SPSS 13.0软件进行统计分析。结果经过规范化培训,观察组医护人员手术开始1、2、3h时手细菌培养菌落数分别为(0.402±0.016)CFU/cm2、(0.425±0.019)CFU/cm2和(0.485±0.021)CFU/cm2,对照组分别为(0.581±0.017)CFU/cm2、(0.622±0.016)CFU/cm2和(0.796±0.020)CFU/cm2,观察组明显少于对照组;观察组医护人员外科手消毒细化指标执行达标率平均达到93.38%,明显高于对照组75.98%,两组比较差异有统计学意义(P<0.05)。结论通过规范化培训,可以有效提高医护人员外科手消毒细化指标执行达标率,并有利于在手术全过程中均保持较低的手细菌菌落数。

OBJECTIVE To investigate the methods and clinical value of establishing standardized trainings for the department of surgery to realize hand disinfection so as to provide references for controlling the nosocomial infection rate.METHODS Medical care workers before and after the standardized training of hand disinfection in the operation room of surgery department were selected and divided into observation group and control group with 68 cases each.All aspects were made to be comparative.The hand disinfection condition of the two groups was evaluated by refined indexes;hand bacteria culture was conducted at different surgery time and the disinfection condition was evaluated.All data were analyzed by SPSS 13.0software.RESULTS After a standardized training,the hand bacteria colonies in the observation group at the 1h,2hand 3hafter the surgery were（0.402±0.016）CFU/cm^2,（0.425 ± 0.019）CFU/cm^2 and（0.485 ± 0.021）CFU/cm^2;the control group were（0.581 ± 0.017）CFU/cm^2,（0.622±0.016）CFU/cm^2 and（0.796±0.020）CFU/cm^2.The bacteria colonies in the observation group was significantly less than that of the control group;the average control rate of the refined indexes in the observation group was 93.38%,which was obviously higher than the control group with the rate of 75.98%.The difference was significant（P〈0.05）.CONCLUSIONThe standardized trainings can effectively improve the condition that medical workers implement the refined indexes of hand disinfection in surgery department and hence are benefit for the controlling of hand bacteria colonies during the whole processes of surgeries.
