血清IGF-I、bFGF水平与早产儿视网膜病的关系

摘要 目的探讨血清中胰岛素样生长因子-I(IGF-I)、碱性成纤维细胞生长因子(b FGF)预测早产儿视网膜病(ROP)的诊断价值。方法胎龄≤31周和(或)体质量≤1500g的早产儿进行间接眼底镜筛查,根据ROP国际分类法诊断为ROP的患儿为实验组,其中根据严重程度分为ROP I-IV期;对照组:选择同期与ROP组胎龄、出生体质量相匹配的非ROP早产儿。对所有研究对象在出生后1周或纠正胎龄31周时取静脉血2m L,采用ELISA法检测血清中IGFI、b FGF浓度。结果随着ROP的进展,血清中IGF-I及b FGF逐渐降低;通过ROC曲线分析发现,IGF-I的最佳阈值为28.08ng/m L,灵敏度为0.94,特异度为0.812,ROC曲线下最大面积为0.902;b FGF的最佳阈值320.65ng/L,灵敏度为0.84,特异度0.854,ROC曲线下最大面积为0.896。由此可见,IGF-I具有较好的预测和诊断价值,b FGF次之。结论临床上如有胎龄≤31周和(或)出生体质量≤1500g的早产儿,在其生后第1周或纠正胎龄31周时,若血清IGF-I明显降低(<28.08ng/m L),同时伴有b FGF降低(<320.65ng/L),则预示ROP的发生率增加,需请眼科医生行眼底筛查。
