结直肠癌患者血清肿瘤标志物联合检测的临床意义

Clinical significance of combined detection of serum tumor markers in patients with colorectal cancer

目的探讨血清肿瘤标志物联合检测在结直肠癌（CRC）患者病情监测中的临床意义。方法 116例CRC患者根据不同肿瘤分期分为Ⅰ~Ⅱ期组50例和Ⅲ~Ⅳ期组66例,比较2组血清肿瘤标志物水平。同时比较化疗后12个月和24个月时复发转移与未复发转移者血清肿瘤标志物水平差异。结果Ⅰ~Ⅱ期组患者血清癌胚抗原（CEA）、糖链抗原19-9（CA19-9）和CA242水平均显著低于Ⅲ~Ⅳ期组,差异有统计学意义（P〈0.05）;2组血清甲胎蛋白（AFP）、CA724水平比较,差异无统计学意义（P〉0.05）;化疗后12个月,复发转移与无复发转移患者血清CEA、CA199、CA242、CA724水平比较,差异有统计学意义（P〈0.05）;2组血清AFP水平比较,差异无统计学意义（P〉0.05）;化疗后24个月,复发转移与无复发转移患者血清CEA、CA199、CA242、AFP、CA724水平比较,差异有统计学意义（P〈0.05）。结论血清肿瘤标志物联合检测在评估CRC患者病情、监测化疗后是否复发转移上具有一定临床价值。

Objective To explore the clinical significance of combined detection of serum tumor markers in patients with colorectal cancer（ CRC）. Methods A total of 116 CRC patients were divided into stageⅠ ~ Ⅱ group（ n = 50） and stage Ⅲ ~ Ⅳ group（ n = 66） according to phase of tumor,and levels of serum tumor markers [including carcino-embryonie antigen（ CEA）,carbohydrate antigen CA19-9（ CA19-9）,CA242,CA724 and alpha fetal protein（ AFP） ]were analyzed. Levels of serum tumor markers after 12 months and 24 months of chemotherapy were compared between patients with relapse or metastasis and patients without relapse or metastasis. Results The levels of CEA,CA199 and CA242 in the stage Ⅰ ~ Ⅱ group were lower than those in the stage Ⅲ ~ Ⅳ group（ P 0.05）. There were no significant differences in levels of AFP and CA724 between two groups（ P 0.05）. After 12 months of chemotherapy,except for AFP,there were significant differences in the levels of CEA,CA199,CA242 and CA724 between patients with relapse or metastasis and patients without relapse or metastasis（ P 0. 05）. After 24 months of chemotherapy,there were significant differences in levels of all the markers mentioned above between patients with relapse or metastasis and patients without relapse or metastasis（ P 0. 05）. Conclusion The combined detection of serum tumor markers is helpful for evaluating biological behavior and prognosis of disease in patients with CRC.
