集束化护理在矽肺伴Ⅱ型呼吸衰竭机械通气患者的应用

Application of intensive nursing of mechanical ventilation patients with silicosis companying by type Ⅱ respiratory failure

目的探讨集束化护理在矽肺伴Ⅱ型呼吸衰竭机械通气患者中的应用效果。方法选取2012年1月—2013年6月本院收治的矽肺晚期伴有Ⅱ型呼吸衰竭患者行机械通气的50例作为观察组,实施人工气道集束化护理。选择2010年3月—2011年12月收治的同科室同类患者55例作为对照组,采用常规护理方法。比较2组患者呼吸机相关性肺炎(VAP)的发生率、机械通气时间、病死率。结果观察组患者VAP发生率、病死率低于对照组,机械通气时间短于对照组,差异有统计学意义(P<0.01)。结论矽肺伴Ⅱ型呼吸衰竭机械通气患者进行人工气道集束化护理可减少VAP发生率、病死率并缩短机械通气时间。

Objective To explore the application effect of intensive nursing of mechanical ventilation patients with silicosis companying by type Ⅱ respiratory failure. Methods A total of 50 mechanical ventilation patients with silicosis companying by type Ⅱ respiratory failure from January2012 to June 2013 in our hospital were in observation group,and implemented intensive nursing of artificial airway A total of 55 patients in the same department from March 2010 to December 2011 were as control group,conventional nursing were applied in these patients. Incidence rate of ventilator associated pneumonia（ VAP）,mechanical ventilation time and mortality rate were compared between the two groups. Results The incidence of VAP and mortality rate were lower in the observation group than that in the control group,the mechanical ventilation time was shorter than that in the control group,the differences were statistically significant（ P 0. 01）. Conclusion Intensive nursing of mechanical ventilation of artificial airway patients with silicosis companying by type Ⅱ respiratory failure can decrease the incidence of VAP and mortality rate and shorten mechanical ventilation time.
