摘要
目的探讨个体化教育路径联合数据库管理模式的实施对于住院中青年2型糖尿病患者疾病认知水平、自护行为及代谢指标、心理状态和生存质量的影响。方法利用便利抽样抽取在丽水市人民医院住院的中青年2型糖尿病患者52例,对其进行系统的教育前评估,参照路径实施个体化糖尿病教育,出院后纳入糖尿病患者数据库管理平台进行随访和管理,比较患者教育前后糖尿病知识掌握程度及随访3个月、6个月、1年患者自护行为状况、焦虑/抑郁状态、综合代谢指标及生存质量等的变化。结果本组患者经教育后糖尿病知识问卷平均分从教育前的(40.6±4.2)分升至(83.1±2.9)分,出院时糖代谢指标控制情况较入院时明显改善,随访1年后焦虑及抑郁患者比例分别从教育前的51.9%、28.8%下降至14.0%、12.0%,经教育后随访1年自护行为得分与教育前比较明显提升,随访期间各项指标仍能保持在相对较好地水平,SF-36生活质量量表中8个维度得分及综合得分均呈上升趋势,差异有统计学意义(P<0.05)。结论教育路径联合数据库管理模式能有效提高住院中青年2型糖尿病患者的疾病认知水平,促使其建立和维持较好的自护行为,改善其综合代谢指标、焦虑/抑郁状态,提高生存质量。
Objective To explore the effects of individualized education path combined database management on cognition,self-care behavior and metabolism,mental state and quality of life among young patients with type 2 diabe-tes.Methods Using convenience sampling method,52 cases of young inpatient with type 2 diabetes were investigated by systematic education evaluation,individualized diabetes education path,then entered into the follow-up and management of database management platform.Their changes of knowledge,self-care behavior,anxiety/depression,comprehensive meta-bolic index and quality of life before and after the education 3 months,6 months,and 1 year were compared.Results The average score was increased from (40.6 ±4.2)to (83.1 ±2.9).Comprehensive metabolic index was increase significant-ly.After 1 year follow-up,the proportion of patients with anxiety was decreased from 51.9% to 14.0%,the proportion of patients with depression was decreased from 28.8% to 12.0%.The self-care behavior improved significantly.8 dimensions of SF-36 score and comprehensive score showed an upward trend,the difference was statistically significant (P 〈 0.05 ). Conclusion Education path combined database management mode can effectively improve the cognitive level of young pa-tients with type 2 diabetes,promote the establishment and maintenance of self-care behavior,and improve their comprehen-sive metabolic index,the status of anxiety and depression,and improve their quality of life.
