早期2型糖尿病肾病患者加用益气活血补肾方治疗前后的肾脏超声动态变化的观察

Renal Ultrasound Dynamic Observation of Early Type 2 Diabetic Nephropathy Patients before and after the Treatment of Yiqi Huoxue Bushen Recipe

目的观察益气活血补肾方治疗早期2型糖尿病肾病患者肾脏超声的动态变化。方法参照MogenSen诊断标准,选择2型糖尿病肾病Ⅲ期患者作为研究对象,对照组给予常规治疗,治疗组在常规治疗的基础上加用益气活血补肾方治疗,治疗时间为4个月。检测患者尿微量白蛋白,作为判断疗效的依据;同时,采用彩色多普勒超声观察肾脏主肾动脉(master renal arteries,MRA)、肾锥体叶间动脉(interlobar renal arteries,IRA)及肾窦部段动脉(segmental renal arteries,SRA)的收缩期最大流速(systolic velocity,Vs)、舒张期最低流速(diastotic velocity,Vd)、血流阻力指数(resistance index,RI)。结果治疗组患者尿微量白蛋白取得更好的控制效果,肾脏彩色多普勒超声提示MRA、IRA及SRA的Vs、Vd、RI改善程度明显好于对照组。结论加用益气活血补肾方治疗可以有效的提高早期2型糖尿病肾病患者的肾脏血流灌注,抑制病情进展。

Objective To investigate the dynamic changes of renal ultrasound results using Yiqi Huoxue Bushen Decoction in the treatment of early type 2 diabetic patients with nephropathy. Methods According to MogenSen diagnosis and staging criteria, phase Ⅲ type 2 diabetic nephropathy patients were studied. The control group was given conventional treatment, and the treatment group was added with Yiqi Huoxue Bushen Decoction. The treatment lasted for 4 months. Trace urinary albumin in patients were detected to evaluate the curative effect, as the basis. At the same time, color Doppler ultrasound was used to observe the renal main renal artery （MRA） , renal pyramid interlobar artery （IRA） and renal sinus segment artery （SRA） and the peak systolic velocity （Vs） , end diastolic velocity （Vd） , resistance index （RI） . Results The treatment of patients with urinary albumin group had better control effect. MRA, IRA and SRA of Vs, Vd, RI were significantly better than that in the control group.Conclusion Treatment combined with yiqihuoxue bushen prescription can effectively improve renal perfusion in patients with early type 2 diabetic nephropathy and inhibit disease progression.
