全科团队模式社区干预对高尿酸血症患者疾病控制有效性及可行性分析

Effectiveness and Feasibility Evaluation of Control Team Mode of Community Intervention on Patients with Hyperuricemia

目的研究高尿酸血症患者实施全科团队模式社区干预对疾病控制的有效性及可行性。方法采用社区随机对照研究的方法,随机选取上海市浦东新区高桥社区8个居委的202个高尿酸血症病人作为研究对象,随机分为干预组(n=104)和对照组(n=98),对干预组病人进行全科团队模式的社区干预,对比两组对所患疾病的认知程度,对比两组血脂、血尿酸、BMI、血压等改变情况。结果干预组对高尿酸血症的认识较对照组有显著提高(P<0.01),干预组自6月开始血尿酸监测较对照组有显著改善(P<0.05),干预组与医生交流、饮食控制、情绪管理、饮酒、高尿酸血症管理的总体自我效能评分、体能锻炼、血压控制、健康自评、健康担忧、疾病对生活的影响、痛风发作次数等方面,较对照组均有显著改善(P<0.05)。结论高尿酸血症患者实施全科团队模式社区干预,能够使患者与医生更直接的沟通交流,及时获取饮食控制及情绪管理的方法,对疾病有更加充分的认识,对疾病控制能够起到积极的推进作用,高效可行,应予推广。

Objective To evaluate the effectiveness and feasibility of the implementation of general team mode of community intervention on the disease control in patients with hyperuricemia.Methods A randomized controlled study in community were performed. A total of 202 hyperuricemia patients randomly selected from 8 Residents＂ committees in Shanghai Pudong New Area Gaoqiao community were researched and randomly divided into the intervention group （n = 104） accepting community intervention and control group （n = 98） . The cognitive degree of risk disease, blood lipid, blood uric acid, body mass index （BMI） , and blood pressure changes were compared between the two groups.Results The awareness of hyperuricemia in the intervention group was significantly higher than that in control （P〈0.01）. The situation of beginning to uric acid monitoring in the intervention group was improved significantly compared with the control （P〈0.05） since June. The communication with doctor, diet, emotional management drinking, hyperuricemia, gen- eral management self-efficacy score, physical exercise, blood pressure control, self-rated health, health concerns, disease influence on life, gout attack frequency and so on in the intervention group were improved significantly compared with the control （P〈0.05）.Conclusion The implementation of general practitioner team mode of community intervention on hyperuricemia patients, can make patients and doctors communicate more effectively, access to methods of diet control and emotional management more timely, understand the disease more fully which can play a positive role in promoting the disease control, and is efficient, feasible and worthy of popularization.
