关节镜结合中药治疗老年人髌股关节炎的临床研究

Clinical Study on Arthroscopic Combined with Traditional Chinese Medicine in Treating Elderly Patients with Patellofemoral Arthritis

目的探讨关节镜结合中药治疗老年人髌股关节炎的疗效。方法将76例82髌股关节炎患者关节镜下行关节清理术后随机分成治疗组和对照组,各38例。对照组术后常规给予引流包扎等处理,行相关功能锻炼。术后1周予玻璃酸钠关节内注射,2 ml/次,每周1次,共5次。治疗组在此基础上加用补肾坚骨汤,1剂/d,分2次温服。对术后1周、1个月、6个月、12个月的疗效分别评定。结果术后1周,治疗组总有效率高于对照组,但两组差异无统计学意义(P>0.05);术后1个月、6个月、12个月治疗组的总有效率均明显高于对照组,差异有统计学意义(P<0.05)。结论关节镜下关节清理术结合中药治疗老年人髌股关节炎疗效持久稳定,值得在临床推广。

Objective To explore the curative effect of arthroscopic treatment combined with traditional Chinese medicine of elderly patients with patellofemoral arthritis.Methods A total of 76 patients after arthroscopic debridement surgery with 82 cases of patellofemoral osteoarthritis were randomly divided into treatment group and control group,with38 cases each.Control group was given conventional dressing processing and corresponding function exercise.Sodium hyaluronate was intra articular injected one week,after operation,2 ml/time,a total of 5 times.Treatment group was additionally treated by Bushen Jiangu decoction,1 dose/d.The curative effect was assessed on1 weeks,1 months,6 months,12 months postoperatively.Results After 1 week,the total effective rate of treatment group was higher than the control group,but not significantly（P〉0.05）.The total effective rate after 1 months,6 months,12 months in treatment group were significantly higher than the control group（P〈0.05）.Conclusion Arthroscopic debridement combined with traditional Chinese medicine has the lasting and stable curative effect in the treatment of elderly patients with patellofemoral arthritis,which is worthy of clinical promotion.
