摘要 目的探讨25-羟维生素D3与哮喘患儿免疫因子的相关性及对免疫因子的影响。方法选取80例8个月~5岁的哮喘患儿,随机分为哮喘1组和哮喘2组,各40例,80例患儿均给予常规治疗,哮喘1组在常规治疗基础上给予30万单位维生素D肌肉注射。并选取保健门诊体检儿童40例作为健康对照组。在哮喘患儿治疗前后及健康儿童体检时采用放射免疫分析法测定血清25-羟维生素D3浓度,采用ELISA法测定血清IL-2、IL-4、IL-8和IL-12的浓度。结果哮喘患儿治疗前血清25-羟维生素D3、IL-2、IL-12浓度显著低于治疗后和健康组,IL-4、IL-8浓度显著高于治疗后和健康组。治疗后哮喘1组与2组患儿的各免疫因子水平均存在统计学差异(P〈0.05,P〈0.01)。哮喘患儿治疗前后的血清25-羟维生素D3浓度与哮喘免疫因子均具有相关性(P〈0.05,P〈0.01)。结论哮喘患儿普遍存在维生素D缺乏,且IL-2、IL-4、IL-8和IL-12等免疫因子水平失调,通过补充维生素D,可使患儿体内维生素D及免疫因子水平均得到改善。 Objective To explore the correlation between the 25-hydroxyl Vitamin D3 and immune factors and its effect on immune factors in children with asthma.Methods A total of 80 children aged between8 months to 5 years with asthma were randomly divided into asthma group1 and 2,with40 cases each.Both groups were given conventional treatment.Asthma group 1 was given 300000 units of vitamin D intramuscular injection based on conventional therapy.Forty healthy children taken clinic physical examination were selected as healthy control group.Before and after asthma treatment or during the physical examination,serum25-hydroxyl Vitamin D3 concentration were tested by radioimmunoassay,the concentration of serum IL-2,IL-4,IL-8 and IL-12 were tested by enzyme-linked immunosorbent assay(ELISA).Results The serum25-hydroxyl Vitamin D3,IL-2,IL-12 concentrations of asthmatic children before treatment were significantly lower,and IL-4,IL-8 concentration was significantly higher than those after treatment and of the control group.After treatment,the levels of the immune factors of two asthma groups were significantly different(P〈0.05 or0.01).Serum25-hydroxyl Vitamin D3 concentrations in children with asthma before and after treatment had significant correlation with asthma immune factors(P〈0.05 or 0.01).Conclusion Vitamin D deficiency,and immune factor IL-2,IL-4,IL-8 and IL-12 disorders is common in children with asthma.The supplement of vitamin D can improve vitamin D and immunity level of these children.
