带蒂颊脂垫结合预制钛板治疗上颌骨肿瘤根治术后面部畸形

Pedicled Buccal Fat Pad Combined with Precast Titanium Plate for Upper Jaw Bone Tumor after Radical Operation with Facial Deformity

目的:探讨带蒂颊脂垫结合预制钛板治疗上颌骨肿瘤根治术后面部畸形的临床价值。方法:选择53例患者,分为两组,其中对照组实施常规的赝复体修复,观察组则使用带蒂颊脂垫结合预制钛板治疗,比较两组患者手术相关情况,术后并发症及3年生存率。结果:观察组手术时间短于对照组(P<0.05),术中出血少于对照组(P<0.05),且术后张口时间显著早于对照组(P<0.05),观察组发生术后感染、发音障碍、面部畸形及吞咽困难的比率均显著低于对照组(P<0.05),观察组3年生存率为66.7%,对照组3年生存率为65.4%。结论:带蒂颊脂垫结合预制钛板治疗上颌骨肿瘤术后面部畸形,其手术效果好,并发症少,且不影响患者术后病情观察以及后续放射治疗(放疗),值得临床重视。

Objective：To explore the clinical value of pedicled buccal fat pad combined with precast titanium plate for upper jaw bone tumor after radical operation with facial deformity.Methods：53 cases were divided into two groups,the control group were received conventional prosthesis,the observation group were treated with prefabricated titanium plate combined with pedicle buccal fat pad,then operation,postoperative complications and 3 year survival rate were compared in the two groups.Results：The operation time in the observation group was shorter than control group（P〈0.05）,the intraoperative bleeding was less than control group（P〈0.05）,and the opening time was earlier than control group（P〈0.05）,the observation group with occurred postoperative infection,ratio of articulation disorders,facial deformity and dysphagia were lower than control group（P〈0.05）,3 year survival rate was 66.7% in observation group,the control group of 3 year survival rate was 65.4%.Conclusion：The operation effect of pedicled buccal fat pad with prefabricated titanium plate in the treatment of maxillary tumor postoperative facial deformity is good and it has less complication and it does not affect the postoperative observation and subsequent radiotherapy,so it is worthy of attention.
