转子周围捆绑植骨加股骨头置换治疗高龄股骨转子间骨折

Objective to in Rotor around the Bundle and Bone Graft with Femoral Head Replacement for Treatment of Femoral Intertrochanteric Fractures in the Elderly

目的:探讨股骨转子周围捆绑植骨加人工股骨头置换治疗高龄股骨转子间骨折临床特点和具体方法。方法:对高龄股骨转子间骨折采用股骨转子周围捆绑植骨加强骨矩及内侧支撑,再按股骨颈骨折的办法,置换人工股骨头。结果:35例患者均顺利通过手术,术后下床时间4-7d,平均6.5d。术后随访3-18个月,平均12.4个月,随访期间,死亡1例,2例再次骨折,32例无假体松动、脱位,X线检查提示植骨区形态良好,25例恢复到受伤前的活动水平。结论:股骨转子周围捆绑植骨对于用人工股骨头置换来治疗股骨转子间骨折,能提供初始稳定性,对维持大、小转子的解剖位子,臀中肌及髂腰肌的张力有确切的意义,恢复髋关节的功能有了保障,远期稳定性需要随访观察。

Vestigate the clinical characteristics of peritrochanteric bundle graft and specific method of bone and artificial femoral head replacement for treatment of femoral intertrochanteric fractures in the elderly.Methods：For elderly patients with femoral intertrochanteric fracture with femoral peritrochanteric bundled bone strengthening bone matrix and the inner support, then the reference Femoral neck fracture, artificial femoral head replacement.Results：35 patients were successfully through the operation, postoperative time 4~7d, average 6.5d. Patients were followed up for 3-18 months, average 12.4 months, the follow-up period, 1 cases of death,2 cases of re fracture,32 cases with no prosthesis loosening, dislocation, X-ray examination showed bone shape is good,25 patients returned to their pre injury activity level.Conclusion：Femoral peritrochanteric bundled bone graft for artificial femoral head replacement for treatment of femoral intertrochanteric fracture, can provide initial stability, to maintain anatomical seat, rotor, have the exact meaning of muscle and iliopsoas hip in tension, restore the function of the hip joint is the guarantee of stability, long term need follow-up observation.
