老年糖尿病妇女骨质疏松的相关危险因素分析 被引量:2

摘要 目的:探讨绝经后2型糖尿病(type 2 diabetes mellitus,T2DM)合并骨质疏(osteoporosis,OP)女性患者骨密度的变化及其相关性。方法:绝经后2型糖尿病女性患被分为2组:T2DM组即T组(62例),T2DM合并骨质疏松组即P组(58例)。比较两组骨代谢、糖代谢等指标之间的差异,及进行相关分析。结果:P组年龄较大,绝经时间长,糖尿病病程长,BMI降低,FBG、HbA1C升高,差异有统计学意义(P<0.05);骨碱性磷酸酶(sBAP)、骨钙素(sOC)、Ⅰ型胶原交联C端肽(sCTx)和尿Ⅰ型胶原交联N端肽(uNTx)明显低于T组。相关性分析显示,患者BMD与糖尿病病程、年龄、HbA1C、FPG、FSH、LH、uNTx/Cr呈显著负相关。结论:绝经后T2DM合并骨质疏松与血糖控制不佳密切相关。 Study changes of bone density about postmenopausal women with type 2 diabetes mellitus (T2DM) and the correlation between osteoporosis (OP) and bone density.Methods:Postmenopausal women were divided into 2 groups: group T2DM,(group T, 62 cases), group T2DM with osteoporosis (P group, 58 cases). Comparison bone metabolism, glucose metabolism between two groups. Results:P group were older, postmenopausal long time, longer duration of diabetes, BMI lower, FBG, HbA1C increased, the difference was statistically signiifcant (P〈0.05). Bone alkaline phosphatase (sBAP), osteocalcin (sOC), type I collagen cross-linked C end peptide (sCTx) and urinary type I collagen cross-linked N end peptide (uNTx) of group P was signiifcantly lower than group T. compared with T group, N group, serum FSH, LH of P group are increased, E2 is decreased (P〈0.05). Correlation analysis showed that BMD is signiifcant negative correlation with and the duration of diabetes, age, HbA1C, FPG, FSH, LH, uNTx and /Cr, and no correlation with sBAP, sOC, sCTx.Conclusion:Osteoporosis of postmenopausal women with type 2 diabetes mellitus (T2DM) is related to poor glucose level.
