超声引导腹横肌平面阻滞在妇科手术中的应用

Study of Ultrasound-guided Transverses Abdominis Plane Block Applying in Gynecologic Surgery

目的:探讨超声引导腹横肌平面阻滞在妇科手术中的应用价值。方法:选取我院从2013年1月到2013年12月门诊收治的153例妇科开腹手术患者做为研究对象。按照入院顺序随机分为治疗组76例与对照组77例,治疗组患者给予全麻复合腹横肌平面阻滞开腹手术,对照组患者在全麻条件下开腹手术。对比两组患者的镇痛效果及不良反应发生情况。结果:治疗组患者的术后24h内镇痛效果优于对照组患者,差异具统计学意义(P<0.05)。治疗组患者术中与术后舒芬太尼使用量显著低于对照组,差异具统计学意义(P<0.05)。结论:通过超声引导腹横肌平面阻滞在妇科手术中的应用,显著减轻术后患者疼痛,定位准确,安全可靠,值得临床广泛推广应用。

To evaluate the curing effect of ultrasound-guided transverses abdominis plane block applying in gynecologic surgery.Methods： 153 cases of gynecological abdominal surgery patients in our hospital from January 2013 to December 2013 were chosen as research objects. According to the order of admission all patients were randomly divided into the treatment group （76 cases） and the control group （77 cases）. Patients in the treatment group received general anesthesia combined transverses abdominis plane block laparotomy, while those that in the control group received laparotomy under general anesthesia only. Analgesic effects and adverse reactions were compared between the two groups.Results：The postoperative 24h analgesia effect observed in the treatment group was better than this in the control group and the difference was statistically significant （P〈0.05）. Postoperative dosage of sufentanil in the treatment group was significantly lower than this in the control group and the difference was statistically significant （P〈0.05）. Conclusion：The Ultrasound-guided transverses abdominis plane block applying in gynecologic surgery could significantly reduce postoperative pain, positioning more accurately, safe and reliable. It is worthy of wider application.
