三维彩色多普勒血管成像技术在胎盘脐带入口异常诊断中的应用价值

The Clinical Application Value of 3D Color Doppler Angiography in Diagnosis of Abnormal Umbilical Cord Entrance in Placenta

目的:探讨三维彩色多普勒血管成像技术(3D-CDA)在脐带异常插入胎盘诊断中的应用价值。方法:对30例中孕期超声检查疑为胎盘脐带入口异常的胎儿运用3D-CDA检查、随访,并与出生后脐带插入胎盘情况对比。结果:3D-CDA诊断胎盘脐带入口异常28例,其中边缘性脐带入口25例,帆状入口3例(其中1例伴血管前置),3D-CDA诊断提示与产后病理结果一致,诊断符合率100%。结论:3D-CDA有利于准确、空间立体的显示脐带插入胎盘处及其脐带走行情况,在胎盘脐带入口异常的诊断中有重要的应用价值。

Objective To explore the clinical value of three-dimensional color Doppler angiography （3D-CDA） in diagnosis of abnormal umbilical cord entrance in placenta. Methods 30 suspected cases of mid-pregnancy with ultrasound examination for abnormal umbilical cord entrance in placenta were examined by 3D-CDA, followed up, and compared with the placenta after birth. Results For 3D-CDA diagnosis of 28 eases of abnormal umbilical cord entrance in placenta, 25 eases were examined with edge entrance of umbilical cord in placenta, and 3 cases with velamentous entrance of umbilical cord （including 1 case with vasa previa）, 3D-CDA diagnosis result was consistent with the postnatal pathological results, which meant that the diagnostic accordance rate was 100%. Conclusions 3D-CDA has the advantage of accurate, three-dimensional display of umbilical cord insertion of placenta, and the running situation of umbilical cord. 3D-CDA has important application value in the diagnosis of abnormal umbilical cord entrance in placenta.
