门诊双相情感障碍100例临床分析

Clinical Phenomenological Analysis of Out-patients with Bipolar Disorder

目的:研究分析门诊双相情感障碍(BPD)患者的临床症状表现以及药物治疗手段。方法:应用自编的调查问卷对被确诊为ICD^10双相情感障碍的100例患者开展研究。结果:本研究对象共计100人,调查结果表明属于双相I型的患者有80人,属于双相II型的患者有20人。本研究I型患者应用心境稳定剂的人数和应用抗精神药物、苯二氮类药物的人数比II型患者要多,II型患者应用抗抑郁药的人数比I型患者要多,同时使用两种或两种以上的药物进行治疗的患者占总人数的72%。通过治疗,门诊双相情感障碍患者中病情相对比较稳定的病人占总人数的80%。结论:门诊双相情感障碍病人中患双相I型的人数比患双相II型的人数多,II型病人比I型患者更加容易出现漏诊。现阶段门诊使用的临床治疗手段及药物跟国内外指南建议使用的方法和药物基本一致,同时使用多种药物进行治疗,大部分的患者都取得较好的临床效果。

Objective To analyze the patient＇s clinical symptoms with bipolar disorder （BPD） and medical treatment options. Methods 100 patients diagnosed with ICD - 10 bipolar disorder by self-made questionnaire were studied. Results The survey results showed that 80 patients belonged to the BPD type Ⅰ, and 20 people belonged to the BPD type Ⅱ. The numbers of patients in BPD type Ⅰ using mood stabilizers, antipsychotic drugs and benzodiazepine class drugs were more than those in BPD typeⅡ, while used less antidepressants. The numbers of patients simultaneous using two or more kinds of drugs had a portion of 72 %. The stable rate for the patients treated by the drugs was 80 %. Conclusion The results showed that the number of out-patients suffering from BPD type Ⅰ was more than that of BPD type Ⅱ, and BPD type Ⅱ patients were more easy prone to be misdiagnosed than BPD type Ⅰ. Out-patient clinical treatments and drugs at this stage has the same procedure comparing with the abroad. By use of multiple drugs at the same time for treatment, the patients have achieved good effects.
