整体护理在非小细胞肺癌化疗患者中的应用

Application of holistic nursing intervention on patients with non-small-cell lung cancer treated with chemotherapy

目的探讨整体护理在非小细胞肺癌(NSCLC)化疗患者中的应用价值。方法选取2012年2月—2014年4月期间的NSCLC患者50例设为观察组,2010年1月—2012年2月期间的NSCLC患者45例设为对照组。2组均采用NP(长春瑞滨+顺铂)或TP(紫杉醇+顺铂)方案化疗,对照组给予常规护理,观察组给予整体护理。结果观察组有效率(RR)为82.00%,对照组RR为77.78%,2组比较差异无统计学意义(P>0.05);2组不良反应发生率差异无统计学意义(P>0.05);观察组护理依从率为94.00%,对照组护理依从率为77.78%,2组比较差异有统计学意义(P<0.05);观察组护理总满意率98.00%,对照组护理总满意率82.22%,2组比较差异有统计学意义(P<0.05);观察组化疗后生活质量评分(FACT-L)中情绪、生活能力、其他因素得分及总分与化疗前差异显著(P<0.01),对照组各项评分均显著下降(P<0.01),2组差异有统计学意义(P<0.01)。结论整体护理可显著改善NSCLC患者化疗后的生活质量,提高护理依从性,值得临床推广。

Objective To evaluate the holistic nursing intervention on patients with nonsmall-cell lung cancer（ NSCLC） treated with chemotherapy. Methods Fifty NSCLC patients were recruited as observation group between Feb 2012 and Apr 2014,another 45 NSCLC patients were included as control group between Jan 2010 and Feb 2012. Both of two groups received NP（ vinorelbine with cisplatin） or TP（ paclitaxel with cisplatin） chemotherapy. Regular nursing and holistic nursing were performed in control group and observation group,respectively. Results There was no significant difference in response rate between two groups（ 82. 00% of observation group vs 7 7. 7 8 % of control group,P〉0. 0 5）,so was the incidence of adverse reaction（ P〉0. 05）. the compliance rate was 94. 00% in observation group and 77. 78% in control group,with a significant difference（ P〈0. 05）. Observation group received a higher satisfaction rate（ 98. 00% vs82. 22%,P〈0. 05）. The emotional well being,social well being,other factors and total score of functional assessment of cancer therapy-lung（ FACT-L） were decreased in observation group（ P〈0.01）,and results of all FACT-L indexes were declined in control group（ P〈0. 01）. Compared with control group,observation group received a better FACT-L outcomes after intervention（ P〈0. 01）.Conclusion Holistic nursing intervention can improve the quality of life for NSCLS patients with chemotherapy and their compliance to nursing.
