摘要
2010年第3版日本胃癌治疗指南仍将腹腔镜下胃切除手术作为研究性的治疗,腹腔镜下胃切除手术安全性的研究、与开腹手术比较的3期临床研究正在实施中。手术的并发症主要有手术技术和机械设备有关的并发症,也存在科学化管理的瑕疵的问题。正确认识腹腔镜下手术的特殊性,把握住技术要点和规律,正确使用此项技术,根据患者的全身情况,手术适应证,合理的选择手术术式。在不断提高腹腔镜下胃切除手术技术的同时强化及提高围手术期管理水准,将会有效降低各种手术并发症。
In the 3rd edition of Japanese Guidelines for Gastric Cancer Treatment in 2010, laparoscopic gastric surgery has been defined as an investigative clinical study. The study of safety of laparoscopic gastric surgery compared with open operation in the 3rd phase trials is under way. Complications of laparoscopic gastric resection are largely related to the surgical skills and equipments as well as defects of scientific management. We should understand the specificity of laparoscopic surgery correctly, seize the main technical points and rules, employ proper skills, and reasonably choose surgical procedures according to the general condition of patients and operative indications. Surgical complications could be effectively controlled based on enhanced laparoscopic skills and strengthened levels of perioperative management.
