摘要
目的探讨抑郁症患者的注意功能障碍及其相应的大脑神经网络信息传输特征变化规律。方法对34例抑郁症患者和51例健康志愿者行定量脑电信息传输特征分析,并通过视觉注意反应时测验和视觉注意保持测验评估视觉注意反应能力。结果抑郁症患者视觉注意反应时间长于对照组,视觉保持测验失误总数、误判数均高于对照组(t分别=4.71、4.56、3.96,P均<0.05)。伴焦虑的抑郁症患者视觉注意反应时测验的反应时间短于不伴焦虑的患者(t=3.20,P<0.05)。抑郁症组的大脑信息平均传输强度指数、平均强度调整周期、平均传输响应指数、平均响应调整周期均低于对照组(t分别=5.77、4.24、6.39、4.89,P均<0.05),且中重度抑郁症患者的上述脑电信息传输特征参数均低于轻度组(t分别=2.41、2.78、2.91、3.25,P均<0.05)。此外,正确数与年龄呈负相关,与受教育年限呈正相关(r分别=-0.41、0.36,P均<0.05);正确数、大脑信息平均强度调整周期、平均响应调整周期与抑郁、焦虑程度呈负相关(r分别=-0.43、-0.36、-0.40;-0.46、-0.44、-0.48,P均<0.05),平均传输强度指数还与抑郁程度呈负相关(r=-0.36,P<0.05)。结论抑郁症患者存在注意功能缺陷,大脑神经网络信息传输特性明显降低。抑郁症患者注意功能的损害受年龄、教育年限以及临床症状的严重程度等因素不同程度的影响。
Objective To explore the characters of attention dysfunction and the changes of regulation of transmission characteristics of the brain neural network in depression sufferers. Methods A total of 34 depression sufferers and 51 nor- mal persons were examined using the quantitative EEG and evaluated attention response by visual attention test and reten- tion test. Results The response time of visual attention test in depression sufferers was significantly longer than the control group. The total errors and the number of wrong judgment of retention test in depression sufferers was also significantly more than the control group (t=4.71,4.56,3.96 ,P〈0.05). The response time of visual attention test in depression patients who combined with anxiety was significantly shorter than that without anxiety (t=3.20 ,P〈0.05 ). EEG showed that the de- pression patients had lower average transmission intensity index, average transmission coordination period, average trans- mission response index and average transmission response coordination period compared with the control group(t=5.77,4.24, 6.39,4.89,P〈0.05) as well as the patients with severe depression compared with patients with mild depression(t=2.41, 2.78,2.91,3.25,P〈0.05). In addition, correct number were negative related with age and positive related with education (r=-0.41,0.36,P〈0.05). The correct number, average transmission coordination period and average transmission response coordination period were negative related with the degree of depression and anxiety(r=-0.43 ,-0.36,-0.40,-0.46,-0.44 ,-0.48,P〈0.05 ). And negative correlation was found between average transmission intensity index of EEG and the level of depression (r=-O.36,P〈O.05). Conclusion The function of the attention of depression was defec- tive and the information transmission characteristics of the brain neural network were decreased obviously.Age, educational level and the severity of clinical symptoms had the influence on attention dysfunction of depression sufferers.
