预注射地佐辛抑制芬太尼呛咳反应的临床观察

摘要 目的:观察预先静脉注射地佐辛对全身麻醉诱导期芬太尼诱发呛咳反应的抑制作用。方法:选择100例 ASAⅠ~Ⅱ级择期手术全身麻醉患者,随机分为地佐辛组和对照组(0.9%生理盐水),每组各50例。两组患者在麻醉诱导前10 min 经外周静脉分别静脉滴注地佐辛0.1 mg/ kg(用生理盐水稀释至1 mg/ ml)和等容量的生理盐水。麻醉诱导时所有患者均在3 s 内静脉滴注芬太尼4μg / kg。观察并记录静脉滴注芬太尼后1 min 内呛咳反应发生率及发生时间。结果:地佐辛组呛咳反应发生率明显低于对照组(P﹤0.05),且呛咳反应发生时间晚于对照组,两组比较,差异有统计学意义(P ﹤0.05)。结论:预先静脉注射地佐辛0.1 mg/ kg 可有效降低麻醉诱导期芬太尼呛咳反应的发生率。 Objective To observe the inhibition effect of premedication with intravenous dezocine on fentanyl induced cough. Method 100 ASAI ~ II patients undergoing elective operation with general anesthesia were randomly divided into dezocine group and the control group(0. 9% saline solution)with 50 cases in each group. Two groups of patients before induction of anesthesia 10 min received dezocine 0. 1 mg/ kg(diluted with normal saline to 1mg/ ml)or equal normal saline by peripheral vein. Anesthesia was induced in all patients with intravenous injection of fentanyl 4 μg / kg within 3 s. The cough frequency and onset time within 1min after intravenous injection of fenta-nyl. Results The incidence of cough in dezocine group was significantly lower than that of control group(P ﹤0. 05),and the cough onset time in dezocine group was significantly later than that of control group,th edifferece was statistically significant ( P ﹤ 0. 05 ) . Conclusion Premedication with intravenous dezocine 0. 1mg/ kg can effectively reduce the incidence rate of irritating cough caused by fenta-nyl in general anesthesia induction.
