超声征象在诊断BI-RADS 4级乳腺病变中的临床价值

Clinical value of ultrasound findings for diagnosis of BI-RADS 4 breast lesions

目的:分析并探讨超声征象在诊断BI-RADS 4级乳腺病变中的临床价值。方法 :选取经超声检查的BI-RADS4级乳腺外科患者368例(病灶412个),对比超声检查结果与病理检查结果差异。结果:超声检查后,412个病灶中BI-RADS 4A级246个,其中病理结果良性率为88.21%;BI-RADS 4B级105个,其中病理结果良性率为72.38%;BI-RADS 4C级61个,其中病理结果良性率为21.31%。不同超声征象敏感度与特异度有所差异,其中毛刺征、后方回声衰减是特异度较高的超声征象,特异度分别为98.37%、90.20%。结论:超声的不同征象对单独诊断BI-RADS 4级乳腺病变有一定的诊断价值,但并不具有特异性,需要结合其他征象和临床表现才可以综合提高诊断准确率,值得临床借鉴。

Objective To analyze the clinical value of ultrasound findings for the diagnosis of BI-RADS 4 breast lesions.Methods Ultrasound examination was carried out for 368 patients with BI-RADS 4 breast diseases involving 412 lesions,and then the results were compared with those by pathological examination. Results Of all the 412 lesions, there were246 BI-RADS 4A ones with the rate for benign pathological results being 88.21%, 105 BI-RADS 4B ones with the rate being 72.38%, 61 BI-RADS 4C ones with the rate being 21.31%. Ultrasound findings varied in sensitivity and specificity. Spicule sign and posterior echo attenuation were two kinds of ultrasound findings with high specificities of98.37% and 90.20% respectively. Conclusion Ultrasound findings may of value for the diagnosis of BI-RADS 4 breast lesions, although behave not so well in specificity, and thus have to be combined with other measures to enhance the accuracy.
