中医综合护理对妇产科患者术后尿潴留及生活质量的影响

Clinical Research on the Effect of Living Quality for Treating Patients of Obstetrics and Gynecology Postoperative Urinary Retention with Integrated Nursing of TCM

目的:观察中医综合护理对妇产科患者术后尿潴留的治疗效果,并探讨其对患者生活质量的影响。方法:选择2012年4月—2014年3月就诊于本院的妇产科术后尿潴留患者82例为研究对象,将患者随机分为对照组和治疗组各41例。对照组患者给予西医常规病房护理,并联合给予听流水声、膀胱区按摩等排尿诱导,药物选择甲硫酸新斯的明0.5 mg/d臀部肌肉注射;治疗组患者在此基础上给予个体化宣教、心理、亲情支持等综合护理干预,并联合给予中药汤剂内服、针灸综合治疗。治疗后观察两组患者自行排尿时间及膀胱残留尿量,用QLQ-C30生活质量核心量表对患者的生活质量进行评价,计算临床治疗的有效率。结果:治疗后治疗组换的自行排尿时间与对照组有明显缩短,残留尿量较对照组明显减少,两组比较差异具有统计学意义(P<0.05);治疗后治疗组患者的生活质量相关评分即躯体功能、认知功能、角色功能、社会功能、情绪功能评分与对照组比有明显升高,两组比较差异具有统计学意义(P<0.05);对照组有效率为73.1%,治疗组有效率为92.7%,治疗组的有效率明显高于对照组,两组有效率比较,差异有统计学意义(P<0.05)。结论:中医综合护理能有效改善患者的尿潴留症状,缩短患者的排尿时间,提高患者术后的生活质量,效果显著,值得临床广泛推广运用。

Objective：To investigate the clinical efficacy of treating patients of obstetrics and gynecology postoperative urinary retention with integrated nursing of TCM,and observe the effect of living quality.Methods：82 obstetrics and gynecology postoperative urinary retention cases were randomly divided into a control group of 41 cases and a treatment group of 41 cases from April2012 and March 2014.Patients in the control group were treated with routine nursing,listening to the sound of water and bladder region massage.Meanwhile,Patients in the control group were given methyl-sulphate neostigmine 0.5mg / d for buttocks intramuscular injection; while patients in the treatment group were treated with comprehensive nursing intervention,such as education,diet,psychological intervention and family support; Meanwhile,Patients in the treatment group were given self-made decoction for oral treatment,acupuncture and moxibustion.We observed the voidingtime and bladder residual urine volume of the two groups after treatment,evaluated the patients＇ living quality,and calculated the effective rate of the two groups.Results：The voidingtime of the treatment group after treatment was shorter than that of the control group,and the bladder residual urine volume of the treatment group after treatment was less than that of the control group,the difference is significant（P〈0.05）; The life quality score such as physical function evaluation,cognitive function score,role function evaluation score,social function score and emotional function score were improved clearly,the degree of change in the treatment group was more pronounced,the difference is significant（P〈0.05）.The efficiency for patients in the control group is 73.1%,while for patients in the treatment group is 92.7%.The clinical efficacy of the treatment group was higher than that of the control group,the difference is significant（P〈0.05）.Conclusion：Treating patients of obstetrics and gynecology postoperative urinary retention with integrated nursing of TCM can effectively alleviate the uncomfortable symptom after surgery,shorten the time of the patient＇s urine,and improve the quality of life in patients with postoperative,which have the satisfactory clinical effect,is worthy of further research and development.
