摘要
目的观察上颌窦内提升术后种植体植入存留情况以及种植体周围骨量变化情况,为临床应用该技术提供可靠依据。方法选择我科行种植修复患者110例,根据是否行上颌窦提升分为2组。试验组采用上颌窦内提升方式进行上颌窦提升并植入Bio-Oss骨粉,同期进行种植体植入。对照组采用常规操作植入种植体。所有入组患者术后不同时间点进行全口曲面断层检查,牙周探诊深度检查(probing pocket depth,PPD)、探诊出血指数(bleeding on probing,BOP)、菌斑指数(plaque index,PI)检查,种植体存留情况,种植体根方至窦底实际骨高度(actual bone hight,ABH)。应用SPSS15.0统计学软件包进行数据统计学分析。结果试验组55例,植入76颗种植体;对照组55例,植入70颗种植体。2组修复后复查,种植体脱落率、PPD深度、BOP指数、PI指数差异均无统计学意义(P<0.05)。试验组ABH术后即刻与术后36个月相比差异有统计学意义(P<0.05)。结论上颌窦内提升技术为种植体植入骨量不足的患者提供了良好的治疗方案,且能保证种植体较高的存留率,以及良好的植体周围健康。
Objective To evaluate the sur vival rate ofdental implants and changes ofperi-implant stat us after maxillar y sinus lifting. Methods Patients were separated into experimental group and control group according to the fact whether maxillar y sinus floor elevation was placed or not. In experimental group, patients were placed implants simultaneously after maxillary sinus floor elevation and bone graft. In control group, implants were placed directly without maxillary sinus lifting and bone graft. X ray was taken and probing pocket depth(PPD), bleeding on probing(BOP) were measured and plaque Index(PI) ofall implants had been placed on the day ofoperation, 6, 12 and 36 months later. The survival rate ofimplants was counted. The actual bone hight(ABH) from implant root to the bottom ofthe maxillary sinus was calculated. Statistical analysis with SPSS 15.0 package was carried out. Results In experimental group, 55 patients were placed 76 implants. In control group, 55 patients were placed 70 implants. No significant differences were observed in the survival rates, PPD, BOP, PI in two groups(P〈0.05). ABH changes showed obvious bone resor ption(P〈0.05). Conclusion This study showed maxillary sinus floor elevation was a good technique for patients with maxillary bone deficiency. High survival rate and healthy peri-implant status could be ensured.
