Management and Effectiveness of Preventive Antibacterial Drugs for Aseptic Operation

目的分析解放军307医院清洁手术预防性使用抗菌药物情况,包括使用率、用药时机、用药时长等指标的变化趋势,为建立抗菌药物合理使用的长效机制提供参考。方法提取HIS系统中2009年1月至2014年5月期间该医院外科清洁手术患者抗菌药物使用记录,计算相关指标进行分析。结果清洁手术抗菌药物预防使用率从等级医院评审前的70%下降到50%,髋关节置换术、膝关节手术的使用率也呈稳定下降趋势;但子宫肌瘤切除术下降至40%后急剧反弹至80%,其使用时长从之前的2.47 d缩短至0.16 d,反映其用药策略发生了较大变化。结论随着等级医院迎检工作的开展,医院采取了一系列合理使用抗菌药物的举措,使各项指标均有明显改善,并呈持续改进的趋势,反映了控制措施得力,收效明显。

Objective To analyze the use of antibacterial drugs for aseptic operations in a hospital,involving the usage,the timing and duration of use as well as the trend of indices. Methods The drug-use records of surgical patients in a hospital from Jan 2009 to May 2014 from HIS system were retrieved,and the relevant indices were computed and analyzed. Results The usage of antibacterial drugs was reduced to 50% from 70% after the hospital grade evaluation. The usage of THR and knee surgery also exhibitsed the trend of steady decrease,but the rate of myomectomy rebounded to 80% after a decrease to 40%,and the duration dropped to 0. 16 day from 2. 47 days,which mirrors a dramatic change of the strategy. Conclusion With the progress of hospital grade evaluation,hospitals have adopted a series of measures for rational use of antibacterial drugs. As a consequence,relevant indices are improved significantly,which reflects the effectiveness of management measures.
