脑转移瘤患者放疗后生存状况及影响因素分析

Factors influencing survival status of patients with tumor metastasis brain after radiotherapy

目的探讨脑转移瘤患者放疗后生存状况及影响因素。方法回顾性分析接受放疗的脑转移瘤100例患者资料,通过随访记录患者的生存状况,研究患者的年龄、性别、肿瘤转移数目、病理类型、原发灶控制、Karnofsky体力状况评分(KPS)和血红蛋白水平,并进行单因素和多因素分析。结果治疗4个月后,完全缓解13例,部分缓解65例,无明显变化11例,恶化11例。颅外转移、脑瘤个数、原发性损伤控制情况、KPS评分和血红蛋白水平是影响患者1年生存率的相关因素。颅外转移、脑瘤个数、原发性损伤是否控制、KPS评分是转移瘤患者放疗1年生存率的独立影响因素。结论颅外转移、脑瘤个数、原发性损伤是否控制、KPS评分为脑转移瘤患者放疗后生存状况的影响因素。

Objective To explore the related factors influencing the survival status of patients with tumor metastasis brain after radiotherapy. Methods A retrospective analysis the data of 105 cases with tumor metastasis brain after radiotherapy, recording of the patient＇s survival condition through the follow-up. Analysising of age, gender, tumor metastasis, pathological type, the number of primary tumor control, Kan^ofsky performance status （KPS） score, hemoglobin content to determine the relevant factors influencing survival status by single factor analysis and to determine the independent influencing factors by multi-factor analysis. Results Four months after treatment, 13 eases with complete tumor remission, 65 patients with partial remission, 11 cases had no obvious change, 11 patients with tumor progression. Extracranial metas- tasis, tumor is more than 1, the primary injury without effective control, KPS 〈 70, hemoglobin content of less than 10 g/dl is influencing factors related to the 1-year survival rate of patients. Extracranial metasta- sis, tumor number, primary injury whether controlled, KPS score were independent factors related to the l- year survival rate of patients. Conclusion Extracranial metastasis, tumor number, primary injury whether controlled, KPS score were independent factors related to survival status of patients with tumor metastasis brain after radiotherapy.
