摘要
目的探究甲状腺微小癌的超声影像特点及应用价值。方法对2010年1月至2014年6月收治的97例甲状腺微小癌患者(122个癌结节)的超声诊断表现进行回顾性分析,并将其设为观察组。同时纳入100例同期被证实为良性结节患者(120个良性结节)设为对照组,对两组患者的超声影像特点进行比较。结果前后径与横径比≥1、淋巴结血流丰富、低回声、微小钙化等超声表现率,观察组均显著高于对照组,差异均有统计学意义(P<0.05)。超声对97例甲状腺微小癌患者的诊断灵敏度、特异度、阳性预测值、阴性预测值分别为86.89%、83.33%、84.13%、86.21%。结论超声对甲状腺微小癌的敏感性高,有较准确的诊断率,具有重要的临床诊断及鉴别意义。
Objective To explore the characteristics of imaging features of ultrasound for thyroid microcarcinoma and to evaluate the diagnostic significance of application of ultrasound. Methods The clinical data of 97 patients with thyroid microcarcinoma( 122 thyroid microcarcinoma nodules) during January 2010 to June 2014 in this hospital were retrospectively analyzed,and they were allocated in observation group. At the same time,100 patients with benign nodules( 120 benign nodules) were selected as control group. The imaging features of ultrasound in these two groups were compared. Results The rate of ultrasound with aspect ratio ≥1,rich in blood supply,hypoechoes,microcalcification in patients of observation group were significantly higher than those of patients in control group,and the difference was statistically significant( P〈0. 05).The diagnostic sensitivity,specificity,positive predictive value and negative predictive value of ultrasonography in patients with thyroid microcarcinoma were 86. 89%,83. 33%,84. 13% and 86. 21% respectively. Conclusion Ultrasonography has higher sensitivity in diagnosis of thyroid microcarcinoma,and it has higher accurate diagnostic rate,hence it is important in diagnosis and differential diagnosis of this disease.
