住院患者护理风险预警控制体系的建立及应用效果评价

The development and effectiveness of an inpatient nursing risk warning and control system

目的规避住院患者护理风险,减少护理差错的发生,提高患者安全。方法建立护理风险预警控制体系,具体包括建立安全监控网络、应用风险筛选和预警工具、设置风险控制制度与流程。结果临床护士在预防压疮、导管、跌倒/坠床的风险防控措施的达标率提高,上报的护理差错事件数目较实施前减少,上报的用药近似差错数目较实施前比较,差异有统计学意义(χ2=92.42,P<0.01)。结论住院患者护理高危风险预警预控体系能够有效规避风险,提高护士风险防控能力,保障住院患者安全,实现护理质量持续改进。

Objective To reduce nursing risks and errors,and improve patient safety. Methods The nursing risk warning and control system was developed and applied in clinical practice,including the safety monitoring network, tools of risk screening and warning tools,risk control system and process. Results Nurses＇ qualified rate in the risk control of pressure ulcers,catheters and falls were improved（P〈0.01）. The number of reported nursing errors decreased,and the proportion of reported medication errors was reduced（P〈0.01）. Conclusion The inpatient nursing risk warning and control system can effectively prevent nursing risks,improve nurses＇ risk prevention abilities,ensure patient safety and continuously improve the quality of nursing care.
