环孢素注射液致过敏反应1例

One case of allergic reaction caused by cyclosporine injection

1例31岁男性患者,确诊急性髓系白血病4个月,既往有维生素K1过敏史。行异基因外周血造血干细胞移植术,治疗期间使用环孢素注射液预防移植物抗宿主病(GVHD)治疗,输注该药20 min后,患者出现面颈部发红、心慌、气喘、呼吸短促等症状。立即停止输入环孢素注射液,给予抬高床头,吸氧,氧流量3 L·min-1,1 h后症状好转。次日为明确患者对环孢素注射液是否能耐受,再次给予环孢素注射液缓慢静滴,20 min后再次出现上述症状,改用他克莫司抗排异治疗,直至出院未再出现过敏反应。

One 31-year-old male patient with allergic history of vitamin K1 was diagnosed with acute myeloid leukemia 4 months ago. He accepted allogeneic peripheral blood hematopoietic stem cell transplantation. Cyclosporine injection was applied to prevent graft-versus-host disease （GVHD）. Twenty minutes after the infusion of cyclosporine injection, the patient appeared the face and neck redness, lfustered, asthma, shortness of breath and other symptoms. Cyclosporine injection was stopped immediately and the head of the patient was elevated. After the inhalation of oxygen with 3 L·min-1, the patient felt better. The next day he was given cyclosporine injection again to test whether the patient tolerated cyclosporine. The same symptoms appeared again. So tacrolimus was used to prevent GVHD. No allergic reactions happened until he was discharged from the hospital.
