内镜介入联合质子泵抑制剂三联疗法对消化性溃疡伴上消化出血的止血效果

Hemostatic Effect of Endoscopic Intervention Combined with Proton Pump Inhibitor Triple Therapy on Peptic Ulcer Complicating Upper Gastrointestinal Hemorrhage

目的观察内镜介入联合质子泵抑制剂三联疗法在消化性溃疡伴上消化道出血患者治疗中的应用效果。方法随机抽取2012年5月至2014年5月于我院接受治疗的90例消化性溃疡伴上消化道出血患者作为研究对象,按照单双号将其分为两组各45例。对照组仅给予常规质子泵抑制剂三联疗法治疗,观察组在对照组的基础上给予内镜介入疗法,对比两组患者的治疗效果,观察不良反应,比较其止血情况。结果观察组总有效率为97.8%,优于对照组的68.9%,差异有统计学意义(P<0.05);观察组12 h止血率为75.6%,1 d后止血率为86.7%,2 d后止血率为97.8%,均明显高于对照组12 h止血率51.1%、1 d后止血率55.6%和2 d后止血率66.7%,差异具有统计学意义(P<0.05)。结论在消化性溃疡伴上消化道出血患者的临床治疗中,采用内镜介入联合质子泵抑制剂三联疗法,疗效显著,止血率高,值得推广。

Objective To observe the application effect of endoscopic intervention and proton pump inhibitor （PPI） triple therapy on peptic ulcer complicating hemorrhage of upper digestive tract. Methods 90 cases of patients with peptic ulcer complicating hemorrhage of upper digestive tract treated in our hospital from May 2012 to May 2014 were selected and divided into two groups according to the odd and even number method, with 45 cases in each group. The control group only received conventional PPI triple therapy, while the observation group received endoscopic interventional therapy based on the control group. The treatment effect and hemostasis condition were compared and adverse reactions were observed. Results The total effective rate of observation group was 97.8%, better than 68.9%of control group, with statistical difference （P〈0.05）;the hemostasis rate after 12 h of observation group was 75.6%, the hemostasis rate after 1 d was 86.7%, the hemostasis rate after 2 d was 97.8%, higher than 51.1%, 55.6% and 66.7% of control group, with statistical difference （P 〈0.05）. Conclusions In the treatment of patients with peptic ulcer complicating upper gastrointestinal hemorrhage, endoscopic interventional therapy combined with PPI triple therapy has significant effect and high hemostatic rate, which is worthy of promotion.
