ET-1与NT-proBNP浓度变化对高血压左心室肥厚的诊断价值 被引量:4

Diagnostic Value of Plasma ET-1 and NT-proBNP of Patients with Hypertensive Left Ventricular Hypertrophy

摘要 目的:探讨高血压左心室肥厚(Left ventricular hypertrophy,LVH)患者血浆内皮素(Endothelin,ET-1)、氨基末端脑钠肽前体(N-terminal pro brain natriuretic peptide,NT-proBNP)的浓度变化及其诊断价值。方法:收集2011年3月-2013年5月我医院确诊为高血压LVH患者128例设为观察组,选取同期128例体检正常人作为对照组。分析比较两组ET-1、NT-proBNP的浓度。结果:观察组血浆NT-proBNP及ET-1浓度分别为(6.4±3.5)pg/m L和(18.4±1.5)pg/m L,显著高于对照组,差异有统计学意义(P<0.05);ROC图显示,NT-proBNP曲线下面积为0.932,可信区间为0.886-0.988,灵敏度为97.8%,特异度为79.9%;血浆ET-1曲线下面积为0.798,可信区间为0.709-0.892,灵敏度为65%,特异度为93.1%。结论:高血压LVH患者NT-proBNP及血浆ET-1水平显著高于正常人,对高血压LVH患者NT-proBNP及血浆ET-1的诊断具有重要意义。 Objective: To investigate the concentration and its diagnostic value of endothelin (ET-1), N-terminal pro brain natriuretic peptide (NT-proBNP) in plasma of hypertensive left ventricular hypertrophy (LVH) patients. Methods: 128 cases of hypertension patients with LVH from March 2011 to May 2013 in our hospital were enrolled as the observation group, and 128 cases of healthy people during the same period were selected as control group. The plasma concentration of ET-1, NT-proBNP was tested using enzyme-linked immunosorbent assay. Results: The plasma concentration of NT-proBNP and ET-1 in observation group was (6.4± 3.5) (pg/mL) and (18.4± 1.5) (pg/mL) respectively, significantly higher than that of the control group (P 〈0.05). The area under the ROC curve of NT-proBNP in LVH diagnosis was 0.932, with the confidence interval between 0.886 and 0.988, the sensitivity of 97.8% and specificity of 79.9%. The area under the ROC curve of plasma ET-1 in LVH diagnosis was 0.798, with the confidence interval between 0.709 and 0.892, and a sensitivity of 65% and specificity of 93.1%. Conclusion: The plasma level of NT-proBNP and ET-1 of LVH patients with hypertension was significantly higher than that in healthy subjects, which is significant to the diagnosis of LVH patients with hypertension.
