针刺联合促排卵对痰湿型多囊卵巢综合征不孕妇女妊娠率和排卵率的影响

Effect of acupuncture combined with promoting ovulation treatment on pregnancy and ovulation rate of infecundity women with polycystic ovary syndrome of phlegm wet type

目的评价针刺联合促排卵治疗对痰湿型多囊卵巢综合征(PCOS)不孕妇女妊娠率和排卵率的影响,并探讨其作用机制。方法采用随机数字表法将60例PCOS患者分为2组:治疗组30例给予针刺联合口服克罗米芬治疗,对照组30例给予口服克罗米芬治疗。2组均连续治疗4个月经周期,如果期间妊娠试验阳性,则停止针刺与药物治疗。测定2组治疗前后空腹血糖(FBG)、空腹胰岛素(FINS)、血清绒毛膜促性腺激素(HCG)、卵泡刺激素(FSH)、黄体生成素(LH)、睾酮(T)、雌二醇(E2)、孕酮(P)水平,观察2组月经、体质量指数(BMI)、基础体温(BBT)变化及妊娠等情况。结果治疗组总有效率、排卵率、妊娠率均明显高于对照组(P均<0.05);2组患者BMI、胰岛素抵抗均有改善,并且患者性激素水平趋于正常,但治疗组改善情况优于对照组(P均<0.05)。结论针刺联合促排卵治疗可明显改善PCOS患者BMI、胰岛素抵抗及激素水平,提高排卵率及妊娠率,疗效优于单纯药物治疗。

Objective It is to evaluate the effect of acupuncture combined with promoting ovulation treatment on pregnancy and ovulation rate of infecundity women with polyeystic ovary syndrome （PCOS） of phlegm wet type, and to explore its action mechanism. Methods 60 eases of patients with PCOS were divided into 2 groups with random number table method. The treat- ment group with 30 cases were treated with acupuncture combined with clomiphene by oral administration, the control group with 30 cases were only treated with clomiphene by oral administration. Both the groups were treated for four consecutive men- strual cycles. If the pregnancy test was positive during the therapy stage, the treatment was stopped. The levels of fasting plas- ma glucose （FPG）, fasting insulin （FIN）, serum human chorionic gonadotropin （HCG）, follicle-stimulating hormone （FSH） , luteinizing hormone （LH） , testosterone （T） , estradiol （E2 ） and progesterone （P） were determined before and after treatment in both groups, and the menstruation, body mass index （BMI）, basal body temperature （BBT） and pregnancy were observed. Results The total effective rate, ovulation rate, pregnancy rate in treatment group were significantly higher than those in the control group （P all 〈 0.05）. BMI and insulin resistance of the patients in both groups were improved, and the level of sexy hormone became normal, and the improvements in treatment group were better than those in control group（P 〈 0. 01 ）. Conclusion Acupuncture combined with promoting ovulation therapy can significantly improve BMI, insulin resistance and sexy hormone levels in patients with PCOS, increase ovulation rate and pregnancy rate, its curative effect is better than the treatment with medicine therapy alone.
