PKP治疗新鲜和陈旧骨质疏松性椎体压缩骨折 被引量:8

摘要 目的观察并比较经皮椎体后凸成形术(percutaneous kyphoplasty,PKP)对于新鲜和陈旧性椎体压缩骨折的临床疗效。方法自2008年1月至2013年12月收治的42例骨质疏松性椎体压缩骨折患者进行回顾性分析。其中新鲜骨折27例,陈旧骨折15例。两组均给予经皮椎体后凸成形术治疗。对两组术后疼痛缓解、Cobb角度差值、椎体高度改变率进行比较和统计学分析。结果在疼痛缓解方面,术后6~48h新鲜组缓解人数25例(占92.6%),陈旧组缓解人数14例(占93.3%)。两组术后2周的视觉模拟评分与术前比较,差异有统计学意义(P〈0.05);两组间术后2周与术后12个月随访时视觉模拟评分比较差异无统计学意义(P〉0.05)。在椎体高度改变率及Cobb角差值变化方面,新鲜组患者术后椎体前缘改变率和椎体中缘改变率及Cobb角差值均有较大幅度的改善,和陈旧组相比变化明显,两组间比较,差异有统计学意义(P〈0.01)。结论PKP为治疗骨质疏松性椎体压缩骨折的有效方法,其在止痛方面,对于新鲜和陈旧骨折均有良好疗效。但在恢复椎体高度、矫正后凸畸形方面,新鲜骨折疗效明显好于陈旧骨折。因此,对于骨质疏松性椎体压缩骨折的患者,建议尽可能在新鲜期给予PKP治疗。
