原发性自发性气胸病因研究进展 被引量:30

摘要 一、气胸 气胸是指气体在胸膜腔内的异常积聚。胸膜腔在生理状态下是一个呈负压状态的潜在性密闭腔隙,在整个呼吸周期的大部分时间内,其压力均小于体外正常大气压。气体进入胸膜腔可能通过下述三种途径:①胸膜腔直接与体外相通,即创伤性气胸(包括外伤、医源性等),胸壁(皮肤、肌肉和/或肋骨以及壁层胸膜存在破损;②胸膜腔与体内部分器官(这部分器官原存在与外界相通如气管、支气管、肺组织、食管甚至胃肠道)交通,其中最常见病因为胸膜表面肺大疱破裂;③胸膜腔感染,胸腔内滋生产气微生物。气胸可分为自发性气胸和创伤性气胸,前者又可根据是否合并已知肺部疾病或肺组织异常而分为原发性自发性气胸、继发性自发性气胸( Sec-ondary spontaneous pneumothorax, SSP)和其他类型的自发性气胸,后者主要分为医源性和非医源性气胸。
