宫颈环扎联合宫腔镜手术及子宫动脉栓塞术对宫颈妊娠的手术研究

Surgery research of cervical cerclage combined with hysteroscopy surgery and uterine artery embolization on cervical pregnancy

目的：研究宫颈环扎联合宫腔镜手术与子宫动脉栓塞手术在宫颈妊娠手术中的应用效果，为宫颈妊娠手术方案的选择提供可靠依据。方法对本院2013年3月-2014年9月接诊的36例宫颈妊娠产妇进行回顾性分析，随机分为对照组与观察组。对照组18例产妇选用宫腔镜手术联合甲氨蝶呤肌内注射的方案进行治疗，观察组18例产妇选用宫颈环扎联合宫腔镜手术及子宫动脉栓塞术的方案进行治疗。结果相较于对照组，观察组术后血HCG、月经恢复正常时间明显更短，生活质量评分明显更高（P〈0.05）。全体患者均顺利通过手术，对照组与观察组总满意度分别为66.7%、94.4%，差异有统计学意义（P〈0.05）。两组患者术后均未出现发热、腹痛等不适反应。结论宫颈环扎联合宫腔镜手术与子宫动脉栓塞手术在宫颈妊娠手术中有良好的应用价值，值得推广应用。

Objective To study application effect of cervical cerclage combined with hysteroscopy surgery and uterine artery embolization in cervical pregnancy in order to provide a reliable basis for selecting programs for cervical preg-nancy surgery. Methods 36 cases of cervical pregnancy women in our hospital from March 2013 to September 2014 were retrospectively analyzed and women were randomly divided into control group and observation group.Hysteroscopy intramuscular methotrexate for therapy program was used in control group （18 cases）,cervical cerclage combined with hysteroscopy surgery and uterine artery embolization for treatment program were applied in observation group（18 cases）. Results Postoperative blood HCG,menstruation back to normal time in observation group was significantly shorter than that in control group respectively,quality of life score in observation group was significantly higher than that in control group （P〈0.05）.All the women were successful surgery.Total satisfaction rate of observation group and control group was 66.7% and 94.4% respectively,with statistical difference （P〈0.05）.The postoperative had no fever,abdominal pain and other symptoms. Conclusion Cervical cerclage combined with hysteroscopy and uterine artery embolization surgery have good value in cervical pregnancy surgery,it is worthy of promotion and application.
