摘要
目的探究首发抑郁症边缘系统神经细胞的葡萄糖代谢水平及豫驰时间(T2)变化情况。方法采用正电子发射计算机断层显像(PET-CT)扫描及高场MRI(T2WI序列)分别对20例健康受试者(健康组)及20例首发抑郁症患者(首发抑郁组)进行检查,检测边缘系统脑区的最大标准摄取值(SUVmax)及MRI T2值。结果与健康组比较,首发抑郁组患者左侧海马和左侧杏仁核的SUVmax及T2值均降低,差异有统计学意义(P<0.05),右侧海马杏仁核的SUVmax及T2值未出现变化,差异无统计学意义(P>0.05);与健康组比较,首发抑郁组患者左侧前扣带回的SUVmax升高,差异有统计学意义(P<0.05),右侧前扣带回的SUVmax及T2值未见变化,差异无统计学意义(P>0.05)。结论首发抑郁症患者的边缘系统神经细胞受损体现在神经细胞的数目、完整性及对能量的摄取利用上,这些损害是抑郁症发生发展的一个重要机制。
Objective To study the first-episode depression limbic system changes of glucose metabolism of nerve cells and T2 signal changes. Methods Using PET-CT and MRI(T2WI series) to scan 20 first episode depression patients(the first episode group) and 20 healthy examinees(the healthy group),detecting the SUVmax and T2 valuesby MRI of the limbic systemre-spectively. Results Compared to the healthy group,the SUVmax and T2 values of left hippocampus and amygdaloid nucleus of first episode depression patients was reduced significantly,whose difference had statistical significance(P〈0.05).The SUVmax and T2 values of right hippocampus and amygdaloid nucleus had no change. It had no statistical significance in difference (P〉0.05);The SUVmax value of left anterior cingutate of first episode depression patients was increased ,whose difference had statistical signifi cance(P〈0.05). There was no change in the SUVmax of the right anterior cingulate cortex or T2 value by MRI. It had no statistical significance in difference(P〉0.05). Conclusion The nerve cells in limbic system of the first-episode depression patients have been damaged,reflecting in number,integrity of nerve cells and in take and utilization of energy. These damage become an im-portant mechanism of the development in depression.
