摘要
目的:探讨布地奈德、沙丁胺醇联合异丙托溴铵雾化吸入对支气管哮喘急性发作患者临床症状、体征及肺功能的影响。方法:选择符合标准的患者83例,采用入院先后顺序分为观察组42例和对照组41例,两组患者均给予基础治疗,对照组加用沙丁胺醇气雾剂,观察组加用布地奈德气雾剂、沙丁胺醇气雾剂及异丙托溴铵气雾剂,比较两组患者临床症状及体征、肺功能变化情况。结果:观察组患者气急、胸闷、咳嗽及哮鸣音缓解时间明显短于对照组,差异有统计学意义(P<0.05)。两组患者治疗前肺功能相似,差异无统计学意义(P>0.05);治疗后患者肺功能较治疗前均明显改善,差异有统计学意义(P<0.05);观察组较对照组改善幅度更大,差异有统计学意义(P<0.05)。治疗5d后观察组患者临床控制21例、显效12例、好转8例及无效1例;对照组患者临床控制14例、显效13例、好转9及无效5例,差异有统计学意义(P<0.05)。结论:布地奈德、沙丁胺醇联合异丙托溴铵雾化吸入能快速改善支气管哮喘急性发作患者的肺功能,缓解临床症状及体征,临床疗效确切。
Objective: To investigate the effect of aerosol inhalation of budesonide, salbutamol, and ipratropium bromide on the clinical symptoms, physical signs, and lung function in patients with acute epi-sode of bronchial asthma.Method:A total of 83 eligible patients were selected and divided into observation group and control group according to the admission sequence, with 42 patients in the observation group and 41 patients in the control group.The patients of both groups were given with fundamental therapies, but the control group given were with salbutamol aerosol additionally, while the observation group were given with budesonide aerosol, salbutamol aerosol, and ipratropium bromide aerosol additionally, and the changes in clinical symptoms, physical signs, and lung function were compared between the patients of the two groups. Result:The patients of the observation group had significantly shorter times to remission of breath lessness, chest distress, cough, and wheezing sound ( P〈0.05) .The patients of the two groups had similar pre-thera-py lung functions, and the difference was statistically insignificant (P〉0.05).After the treatment, the pa-tients of both groups had significantly improved lung functions compared with those before the ( P〈0.05 );but the patients of the observation group had greater improving extent, and the difference was statistically sig-nificant ( P〈0.05) .5 days after the, the observation group, 21 patients were clinically controlled, 12 pa-tients were responsive to the therapy excellently, 8 patients were improved, and 1 patient was unresponsive to the therapy;while of the control group, 14 patients were clinically controlled, 13 patients were responsive to the therapy excellently, 9 patients were improved, and 5 patients were unresponsive to the therapy;and the differences were statistically significant (P〈0.05).Conclusion:Aerosol inhalation of budesonide, salbuta-mol, and ipratropium bromide can rapidly improve the lung function, and relieve the clinical symptoms and physical signs in patients with acute episode of bronchial asthma, with definite clinical effects.
