电化学发光法检测游离前列腺抗原的效果评价

摘要 目的 探讨电化学发光法检测游离前列腺抗原的效果.方法 选取正常体检者80例.经活检确诊的患有前列腺疾病的患者214例,其中前列腺癌(PCa)79例,前列腺增生(BPH)71例,前列腺炎(BP)64例.采用电化学发光法(ECLA)检测各组患者血清总前列腺特异性抗原(tPSA)、与游离前列腺特异性抗原(fPSA)水平并计算其比值f/tPSA.以fPSA<1 ng/ml为临界点,分析四组患者在fPSA各水平的符合率差异,并计算上述各区间范围内FPSA诊断的灵敏度、特异度、阳性预测值、阴性预测值.结果 前列腺炎组、前列腺增生组患者游离前列腺抗原fPSA、总前列腺抗原tPSA升高,二者比值fPSA/tPSA降低(P<0.05).前列腺癌组的fPSA、tPSA升高更加明显,fPSA/tPSA值也显著降低(P<0.05).正常组与前列腺癌组符合率均为100%,前列腺炎组为93.67%,前列腺增生组为95.77%.总符合率为94.72%.结论 电化学发光法检测游离前列腺抗原灵敏度高,特异性强,适用于临床诊断、鉴别前列腺疾病,值得推广. Objective To study the effect of electrochemical luminescence method to detect free prostate antigen.Methods 80 cases of normal physical examination,214 patients with prostate disease diagnosed by biopsy [79 cases of prostate cancer (PCa),71 cases of benign prostatic hyperplasia (BPH),64 cases of prostatitis (BP)] were selected.Detected the levels of serum total prostate specific antigen (tPSA) and free prostate specific antigen (fPSA) of all subjects by electrochemical luminescence (ECLA),and calculated the ratio of f/tPSA.With fPSA 〈1 ng/ml as critical point,analyzed fPSA coincidence rate of various levels of four groups,and calculated fPSA diagnostic sensitivity,specific,positive predictive value,negative predictive value during different interval ranges.Results fPSA,tPSA of BP group and BPH group rose,fPSA/tPSA lowered (P〈0.05).fPSA,tPSA of PCa group rose more significantly,fPSA/tPSA lowered more significantly (P〈0.05).The coincidence rate of normal group and PCa group were both 100%,that of BP group was 93.67%,that of BPH group was 95.77%,with the total coincidence rate of 94.72%.Conclusions Electrochemical luminescence method to detect free prostate antigen has high sensitivity and strong specificity,which is suitable for clinical diagnosis and differential diagnosis of prostate disease,worthy of clinical promotion.
