局部麻醉下腹膜前无张力疝修补术治疗成人腹股沟疝的体会

Clinical experience of preperitoneal tension-free hernia repair under local anesthesia for adult patients

目的总结分析局部麻醉下腹膜前无张力疝修补术治疗成人腹股沟疝的经验体会。方法回顾性分析2012年7月至2014年6月,南阳市第八人民医院收治的100例成人腹股沟疝患者,应用D10补片局部麻醉下实施腹膜前无张力疝修补术的临床资料。结果 100例患者手术均顺利完成,手术时间40~60 min,术后即可下地活动,术后当天出院,术后口服止痛药1~2次,出院后无需输液及拆线。术后回访3~20个月,无复发患者,无刀口感染,阴囊红肿6例,术区不适感4例,慢性疼痛3例。结论局部麻醉下腹膜前无张力疝修补术是一种简单安全的手术,具有术后恢复快、并发症少、疗效满意等优点。

Objective To summarize the experience of preperitoneal tension-free hernia repair under local anesthesia for patients with inguinal bemia. Methods A total of 100 cases of inguinal bemia in The Eighth People＇s Hospital of Nanyang City who underwent preperitoneal tension-free repair under local anesthesia with D10 mesh from July 2012 to June 2014, were enrolled and retrospectively analyzed. Results All the operations were successfully completed, with the operating time ranged 40 to 60 minutes. Normal activity could be achieved exactly after the operation, and the patients were discharged at the same day. Oral analgesics might be needed 1 to 2 times. Infusion and suture removal were unnecessary. After a follow-up of 3 to 20 months, no recurrence, wound infection and serotal swelling were observed. And there were 4 cases of discomfort sensation and 3 eases of chronic pain in operative area. Conclusion Preperitoneal tension- free hernia repair under local anesthesia is a simple and safe procedure, which has the advantages of rapid recovery, less complications and good outcome.
