亚低温持续治疗时间长短对大鼠脑损伤后颅内压的影响 被引量:5

摘要 目的:分析不同时长亚低温治疗对大鼠脑损伤后颅内压( ICP)的影响,从动物实验角度为临床亚低温治疗提供参考依据。方法通过鼠侧方液压打击脑损伤模型对亚低温治疗时程进行研究,通过颅内压监测并绘制鼠中度脑损伤亚低温治疗颅内压变化曲线图,比较鼠液压侧方打击中度脑损伤亚低温治疗的长时程与短时程对鼠脑组织含水量、颅内压及TNF-α水平影响。结果大鼠脑损伤后颅内压呈上升趋势,5d达到峰值,5~7d开始减少,7d与5d比较差异显著(P<0.05);脑组织含水比率1、3、5、7d分别为(85.34±0.31)%、(83.01±0.24)%、(79.23±0.41)%、(77.10±0.30)%,呈显著下降趋势(P<0.05);大鼠神经功能评分变化趋势与颅内压相同,5d达到峰值,后降低。结论大鼠脑出血后采取亚低温持续治疗效果显著,可降低颅内压,缓解脑水肿,恢复颅内压力,建议选择7d持续治疗方案。 Objective This article was to analyze the effect ofpersistent mild hypothermia therapyon intracranial pressure after traumatic brain injury in rats ( ICP), and provide reference for clinical hypothermia treatmentfrom the angle of animal experiment.Methods Study the hypothermia treatment duration by using the rat lateral fluid percussion brain injury model.Monitoringintracranial pressure and draw the pressure change curve of mild hypothermia therapy of intracranial injury in rat brain.Comparedthe effectslong duration and short duration of hypothermia therapy on rat brain tissue water content and cerebral pressure and TNF-alpha level.Results Intracranial pressure of rat after brain injury rised and reached the peakat the 5thday, 5-7days began to reduce, difference of intracranial pressure at 7thday and 5thday was statistically significant (P〈0.05).Brain water ratio of 1, 3, 5, 7d were (85.34 ±0.31)%, (83.01 ±0.24)%, (79.23 ±0.41)% and (77.10 ±0.30)%respectively, it decreased significant (P〈0.05).Neurological function score variation trendin rats was the same with intracranial pressure.Conclusions The efficacyofpersistent hypothermia therapy on intracerebral hemorrhage rats is significant, it can reduce intracranial pressure, alleviate brain edema and promote intracranial pressure recovery, 7days continuous therapy should be suggested.
