血清胱抑素C和尿微量白蛋白肌酐比在高血压患者早期肾损害筛查中的应用 被引量:3

摘要 目的探讨血清胱抑素C和尿微量白蛋白肌酐比在高血压患者早期肾损害筛查中的应用价值。方法选择尿蛋白阴性的80例原发性高血压患者为高血压组,60例正常健康人为健康对照组。采用免疫投射比浊法检测两组患者的血清胱抑素C(Cys C)含量;免疫浊度法检测尿微量白蛋白含量(m Alb);全自动生活分析仪检测尿肌酐含量(Cr),并计算尿微量白蛋白肌酐比(尿m Alb/Cr)。结果高血压组患者血Cys C含量和尿m Alb/Cr都明显高于对照组(P<0.05);血Cys C和尿m Alb/Cr联合检测检出率为75.0%明显高于血Cys C的60.0%和尿m Alb/Cr的62.5%(P<0.05);血Cys C和尿m Alb/Cr联合检测的灵敏度为75.0%、特异性为91.7%、假阳性率为8.3%、假阴性率为25.0%、诊断符合度为82.1%。结论血Cys C和尿m Alb/Cr联合检测法对高血压早期肾损害患者具有较高的诊断价值。 Objective This paper was to investigate value of serum cystatin C and urinary albumin-creatinine ratio in early renal damage screening for hypertension patients.Methods 80 cases of essential hypertension patients with urinary protein-negative were selected as hypertension group, and 60 cases of healthy people were selected as control group.The serum cystatin C ( CysC ) levels of two groups were detected by immune projection nephelometry method, urinary albumin content ( mAlb) levels of two groups were detected by immune turbidity method.Urine creatinine ( Cr ) levels of two groups were detected by automatic biochemical analyzer.And the urinary albumin-creatinine ratio ( mAlb /Cr) of two groups were calculated.Results Urine CysC and mAlb/Cr levels of hypertensive group were significantly higher control group (P〈0.05).The detection rate of Urine CysC combined mAlb/Cr was 75.0%, which was significantly higher than CysC ( 60.0%) and mAlb/Cr 62.5%(P〈0.05).For Urine CysC combined mAlb/Cr, the sensitivity was 75.0%, specificity was 91.7%, false positive rate was 8.3%, the false negative rate was 25.0%, diagnose compliance was 82.1%. Conclusions Urine CysC combined mAlb/Cr has high value in early renal damage detection for hypertension patients.
