冠心病合并2型糖尿病患者维生素D、胰岛素受体的表达水平及临床意义 被引量:4

摘要 目的探讨冠心病(CHD)合并2型糖尿病(T2DM)患者维生素D、胰岛素受体的表达水平及与其发生发展的相关性。方法 2011年1月至2013年10月四川省人民医院门诊及住院部行冠状动脉造影明确为CHD患者120例(A组),分为CHD合并T2DM(A1组)61例,CHD组(A2组)59例。行冠状动脉造影排除CHD120例(B组),分为T2DM组(B1组)62例,非T2DM组(B2组)58例。入选者均测身高、体质量、腰围(WC)、臀围、血压、总胆固醇(CHO)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、甘油三酯(TG)、空腹血糖(FBG)、餐后2h血糖(2hPPG)、胰岛素α、β受体及25羟基维生素D[25(OH)D],应用方差分析比较各组数据。结果 T2DM患者体质量指数(BMI)较非T2DM组明显增加,差异有统计学意义(P<0.05),CHD组腰围(WC)较其他各组有减小,差异有统计学意义(P<0.05),T2DM组25(OH)D表达水平较非T2DM组降低,CHD组25(OH)D表达水平明显低于非CHD组,差异有统计学意义(P<0.05)。胰岛素α、β受体在T2DM患者较非T2DM患者表达水平均降低,差异有统计学意义(P<0.05);CHD组胰岛素α、β受体较非CHD组血清表达水平下降,差异有统计学意义(P<0.05)。结论 CHD合并T2DM患者维生素D、胰岛素受体的表达水平可能通过高胰岛素血症及胰岛素抵抗等机制来改变,与CHD的发生发展相关。 Objective To investigate the level of 25(OH)D and insulin receptor a,13 in coronary heart disease (CHD) with T2DM. Methods 120 patients with CHD (groupAl:with T2DM; groupA2:without T2DM) and 120 patients without CHD (groupB1 .. with T2DM; groupB2 : without T2DM) were selected. Height, weight, waistline, hip circumference, blood fat, FBG, 2 hPPG, blood pressure, the serum insulin receptor ,β,25 (OH) D were detected. Results The serum levels of Insulin receptor α,β and 25(OH)D in T2DM were all significantly lower than those without T2DM,meanwhile the above parameters in CHD were all significantly lower than those without CHD (P〈 0.05). Conclusion The change in serum level of insulin receptor and 25(OH)D may associate with coronary heart disease though hyperinsulinemia and insulin resistance.
