益气养阴汤治疗慢性萎缩性胃炎随机平行对照研究

Chronic Atrophic Gastritis Parallel Randomized Controlled Study Yiqiyangyin Decoction

[目的]观察益气养阴汤治疗慢性萎缩性胃炎疗效。[方法]使用随机平行对照方法,将110例门诊及住院患者按抛硬币法简单随机分为两组。对照组55例幽门螺杆菌阳性予胶体果胶铋,100mg/次,3次/d,服2周后停;阿莫西林,1g/次,3次/d;奥美拉唑20mg/次,1次/d;克拉霉素,0.5g/次,2次/d。治疗组55例益气养阴汤(丹参、白芍、麦冬、沙参、当归各10g,半夏姜、半枝莲、甘草、枳壳、龙葵各9g,扁豆炒、党参、白花蛇草各15g;胃黏膜脱垂加桔梗、升麻、柴胡、山甲炮各9g;不典型增生加浙贝、山甲炮各10g,生牡蛎15g;肠化生加水蛭、浙贝、莪术各8g,白花蛇草30g;嗳气加旋覆花12g,代赭石20g;泛酸加瓦楞子、乌贼骨各15g;胃酸不足加乌梅、木瓜各12g;十二指肠溃疡加海螵鞘、白芷各10g),1剂/d,水煎200m L,早晚饭前口服。连续治疗60d为1疗程。观测临床症状、腺体萎缩、不良反应。治疗1程,判定疗效。[结果]治疗组显效26例,有效27例,无效2例,总有效率96.36%。对照组显效16例,有效21例,无效18例,总有效率67.27%。治疗组疗效优于对照组(P<0.05)。[结论]益气养阴汤治疗慢性萎缩性胃炎疗效满意,无严重不良反应,值得推广。

Control group, 55 cases of H. pylori positive I colloidal bismuth pectin, 100 mg / times, 3 times / d, serving two weeks after stopping; amoxicillin, 1g / time, 3 times / d; omeprazole 20 mg / times, 1 times / d; clarithromycin, 0.5g / time, 2 times/d.Treatment group 55 cases Yiqiyangyin soup（Danshen,Baishao,Maidong,Shashen, Danggui 10 g, Jiangbanxia, Banzhilian,Gancao,Zhike,Longkui each 9g, Biandou,D angshen,Baihuasheshecao 15g; gastric mucosa prolapse plus Jiegeng, Shengma,Paoshanjia each 9g; dysplasia plus Zhebei,Paoshanjia 10 g, Shengmuli 15g; intestinal metaplasia plus Shuizhi, Zhebei,E＇zhu each 8g, Baihuasheshecao 30g; belching plus Xuanfuhua 12 g, Daizheshi 20g; pantothenic acid plus Walengzi,Wuzeigu each 15g; acid deficiency Jia Wumei,Mugua each 12g; duodenal plus sheath white, Haipiaoxiao,Baizhi 10g）, 1/ d, decoction 200 m L, morning and evening meals orally. 60 d is a continuous course of treatment. Observation of clinical symptoms, inflammation, glandular atrophy, adverse reactions. Treatment of a process to determine efficacy.[Results] treatment group 26 cases, effective 27 cases, 2 cases, the total efficiency of 96.36%. Control group, 16 cases markedly effective in 21 cases, 18 cases, the total efficiency of 67.27%. Treatment group than the control group（P〈0.05）. [Conclusion] Yiqiyangyin Tang treatment of chronic atrophic gastritis effect is significant, it is worth promoting.
