益胃汤联合西药治疗慢性萎缩性胃炎随机平行对照研究

Chronic Atrophic Gastritis Parallel Randomized Controlled Study Yiwei Tang Combined with Western Medicine

[目的]观察益胃汤联合西药治疗慢性萎缩性胃炎疗效。[方法]使用随机平行对照方法,将47例门诊患者按病志号抽签法简单随机分为两组。对照组25例兰索拉唑,30mg/次,3次/d;甲硝唑,400mg/次,3次/d;左氧氟,200mg/次,3次/d。治疗组22例益胃汤(白芍、麦冬各8g,当归、丹参、沙参各12g,黄连6g,半枝莲8g,党参14g,白花蛇舌草12g,甘草、姜半夏各8g,扁豆炒12g;肠上化生加莪术8g,浙贝、水蛭各9g,白花蛇舌草25g;胃黏膜脱垂加柴胡10g,升麻、桔梗各8g,炮山甲10g;炎症、十二指肠球部溃疡加白及8g,白芷12g,海螵蛸8g;胆汁反流加郁金、竹茹、柴胡各10g;腺体不典型增生加炮山甲8g,浙贝、生牡蛎各12g;胃酸缺失加瓜蒌11g,乌梅13g),1剂/d,水煎200m L,早晚口服;西药治疗同对照组。连续治疗14d为1疗程。观测临床症状、肠化增生、腺体萎缩、不良反应。连续治疗3疗程,判定疗效。[结果]治疗组显效13例,有效8例,无效1例,总有效率95.45%。对照组显效9例,有效9例,无效7例,总有效率72.00%。治疗组疗效优于对照组(P<0.05)。症状积分治疗组改善优于对照组(P<0.05)。[结论]益胃汤联合西药治疗慢性萎缩性胃炎疗效满意,无严重不良反应,值得推广。

[Objective]To observe Yiwei Tang combined with Western medicine treatment of chronic atrophic gastritis efficacy.[Methods]Using random parallel control method, 47 cases of outpatients by disease Zhi No. ballot method is simple and randomized into two groups. 25 cases in the control group lansoprazole, 30mg/time, 3 times/d; metronidazole, 400mg/time, 3 times/d; left oxygen fluorine, 200mg/times, 3 times/d. Treatment group 22 cases of stomach soup （Baishao,Maidong each 8g, Danggui,Danshen,Shashen 12g, Huanglian 6g, Banzhilian 8g, Dangshen14g, Baihuasheshecao 12g,Gancao,Jiangbanxia 8g, Chaobiandou 12g; intestinal metaplasia plus e＇zhu 8g, Zhebei,Shuizhi each 9g, Baihuasheshecao 25g; gastric mucosa prolapse plus Chaihu lOg, Shengma,Jiegeng each 8g, Papshanjia 10g; inflammation, duodenal bulbar ulcer with Baiji 8g, Baizhi 12g, Haipiaoxiao 8g; bile reflux add Yujin,Zhuru,Chaihu each 10g; glandular dysplasia plus Paoshanjia 8g, Zhebei,Shengmuli 12g; Lack of gastric acid plus Gua 11g, Wumei13g）, l/d, decoction 200mL, sooner or later, oral; western medicine with the control group. 14d is a continuous course of treatment. Observation of clinical symptoms, endoscopy, intestinal metaplasia, hyperplasia, glandular atrophy, epigastric fullness, abdominal pain, belching, poor appetite, adverse reactions. Continuous treatment for 3 courses, to determine efficacy. [Results] Treatment group 13 cases, 8 cases, ineffective one case, the total efficiency of 95.45%. A control group that were nine cases, effective in 9 cases, 7 cases, the total efficiency of 72.00%. Treatment group than the control group （P 〈0.05）. Symptom score improvement in the treatment group than the control group （P〈0.05）.[Conclusion] Yiwei Tang combined with Western medicine treatment of chronic atrophic gastritis, the effect is significant, it is worth promoting.
