椎弓根螺钉内固定治疗不稳定胸腰椎骨折35例疗效分析 被引量:2

摘要 目的探讨椎弓根螺钉(AF钉)内固定治疗不稳定胸腰椎骨折的临床疗效。方法采用经后路植入AF钉对35例不稳定型胸腰椎骨折患者进行治疗,测量手术前与末次随访时的伤椎高度、后凸角,对合并神经损伤患者进行Frankel评分,评价临床疗效。结果患者随访时间6~24个月,术前与末次随访相比,伤椎平均高度由术前37%~69%恢复到80%~98%;后凸角由术前的20~37°恢复到0~11°;按Frankel分级:A级3例无恢复,B级7例恢复到C级6例,1例无恢复;C级11例均恢复至D级;D级8例恢复到E级6例,2例无恢复。结论 AF钉内固定治疗不稳定型胸腰椎骨折能使伤椎得到良好的复位,重建脊柱的稳定性,促进脊髓损伤患者的神经功能恢复。植入胸椎椎弓根螺钉时风险较大,但只要熟悉胸椎椎弓根解剖特点,选择合适直径椎弓根螺钉,并注意螺钉植入的方向、深度,仍然可以获得满意的临床疗效。
