血清CA72-4、CA199、CEA在结直肠癌患者中的检测及临床意义

Detection and clinical significance of serum CA72-4,CA199,CEA in patients with colorectal cancer

目的：探讨血清糖类抗原72-4（CA72-4）、糖类抗原199（CA199）、癌胚抗原（CEA）在结直肠癌患者中的检测及临床意义。方法：收集82例结直肠癌患者设为结直肠癌组,另收集结直肠癌良性病变患者45例,设为良性病变组,健康体检者30例,设为健康对照组;检测3组受试者血清CA72-4、CA199、CEA含量,比较结直肠癌组不同TNM分期和分化程度间3者的差异,统计分析单项及联合检测诊断结直肠癌的敏感性和特异性。结果：结直肠癌组CA72-4、CA199和CEA水平均高于良性病变组和健康对照组,差异均有统计学意义（P〈0.05）;结直肠癌组TNM分期Ⅲ~Ⅳ期患者CA72-4、CA199和CEA水平均高于Ⅰ~Ⅱ期患者,低分化程度患者高于中、高分化程度患者,差异均有统计学意义（P〈0.05）;CA72-4、CA199和CEA单独诊断结直肠癌的敏感性较低,联合检测可以提高对结直肠癌诊断的敏感性。结论：结直肠癌患者血清中CA72-4、CA199、CEA表达升高,且与疾病的TNM分期和分化程度相关,联合检测对结直肠癌的诊断具有一定的指导意义。

Objective： To explore the detection and clinical significance of serum CA72-4, CA199, CEA in patients with colorectal cancer. Methods A total of 82 cases with colorectal cancer were collected, and were set as coloreetal cancer group. Another 45 patients with colorectal benign lesions were set as benign lesion group, 30 healthy people were set as healthy con- trol group. The serum CA72 4, CA199, CEA levels were detected in three groups, compared them among different TNM staging and differentiation degree of colorectal cancer group,analyzed the sensitivity and specificity of diagnosis Individual and joint detection. Results The levels of CA72-4, CA199 and CEA in colorectal cancer group were significantly higher than that of benign lesion group and healthy control group （P〈0.05）; In the colorectal cancer group, CA72-4, CA199 and CEA levels in patients with TNM stage Ⅲ-Ⅳ were significantly higher than that of Ⅰ-Ⅱ period patients, patients in low degree of differentia- tion were significantly higher than that in high degree of differentiation- （P〈0. 05）; the diagnosis sensitivity of CA72-4, CA199 and CEA separate was low, the joint detection could improve the sensitivity of diagnosis of colorectal cancer. Oonclusions The expression of serum CA72-4, CA199 and CEA in patients with colorectal cancer are significantly increased and areassociated with the TNM staging and differentiation of disease, joint detection in the diagnosis of colorectal cancer has certain guiding significance.
