吉西他滨热化疗灌注联合卡铂化疗栓塞治疗对肝癌患者血清指标的影响

Effect of gemcitabine heat perfusion chemotherapy combined with carboplatin chemotherapy embolization on serum indexes in patients with hepatocellular carcinoma

目的：研究吉西他滨热化疗灌注联合卡铂化疗栓塞治疗对肝癌患者血清指标的影响。方法：将2013年5月~2014年7月期间我院收治的90例原发性肝癌患者纳入研究,随机分为两组,观察组患者接受吉西他滨热化疗灌注联合卡铂化疗栓塞治疗,对照组患者接受吉西他滨常规化疗灌注联合卡铂化疗栓塞治疗,比较两组患者的血清肿瘤标志物含量、黏附分子含量以及Th细胞含量。结果：（1）肿瘤标志物：观察组患者血清高尔基体蛋白73（GP73）、胸苷激酶1（TK1）、分泌型蛋白Dickkopf-1（DKK1）含量低于对照组;（2）黏附分子：观察组患者血P-选择素（P-selectin）、可溶性细胞间黏附分子-1（sICAM-1）、CD44v含量低于对照组;（3）Th细胞含量：观察组患者的Th1细胞含量、Th1/Th2细胞比例均高于对照组,Th2细胞含量低于对照组。结论：吉西他滨热化疗灌注联合卡铂化疗栓塞能够更为有效杀灭肝癌细胞、降低血清肿瘤标志物和黏附分子含量,同时改善机体免疫功能、增加Th1细胞含量,是治疗肝癌的理想方法。

Objective：To study the effect of gemcitabine heat perfusion chemotherapy combined with carboplatin chem- otherapy embolization on serum indexes in patients with hepatocellular carcinoma. Methods.. A total of 90 cases of hepatocellu- lar carcinoma in our hospital from 2013 May - 2014 July were enrolled and randomly divided into two groups. Observation group received gemcitabine heat perfusion chemotherapy combined with carboplatin chemotherapy embolization, control group received gemcitabine conventional perfusion chemotherapy combined with carboplatin chemotherapy embolization. Then serum tumor marker levels, adhesion molecules levels and Th cells content were compared. Results： Serum GP73, TK1, DDK1 con- tent of observation group were lower than those of control group; serum sICAM-1, P-selectin, CD44v content of observation group were lower than those of control group; Thl cell content and Thl/Th2 cell ratio of observation group were higher than those of control group; Th2 ceil content was lower than that of control group. Conclusion： Gemcitabine heat perfusion chemo- therapy combined with carboplatin chemotherapy embolization can effectively kill hepatocellular carcinoma ceils, reduce serum tumor markers and adhesion molecules content and improve immune function, increase Thl cells content; it＇s an ideal methodin treating hepatocellular carcinoma.
