多器官功能障碍综合征患者的凝血功能变化及其意义被引量：1

The Significance of Changes of Coagulation Function of Patients with Multiple Organ Dysfunction Syndrome

摘要目的观察多器官功能障碍综合征患者凝血功能变化,并探讨其与急性生理和慢性健康评分(APACHE II评分)及病情严重程度关系。方法选择本院于2013年1月-2014年4月收治的42例作为研究对象,根据APACHE II评分分为三组,其中A组18例,APACHE II评分10-20分;B组15例,APACHE II评分20-30分;C组9例,APACHE II评分>30分;并根据预后分为生存组29例及死亡组13例,测定42例患者入院时凝血功能相关指标,分析其与APACHE II之间关系。结果 A组PT、a PTT水平分别为(19.71±4.53)s、(54.32±6.31)s,均低于B、C组,均P<0.05;PLT水平三组比较统计学差异显著,均P<0.05;三组TT、Fbg比较无统计学差异,P>0.05。生存组PT、a PTT水平分别为(21.58±4.18)s、(58.76±6.59)s,均低于死亡组,均P<0.05;生存组PLT水平为(108.69±18.74)×109/L,高于死亡组,P<0.05。PT、a PTT及PLT与预后具有相关性,其中,rPT=0.687,ra PTT=0.574,rPLT=-0.437。结论凝血功能相关指标测定有助于评估多器官功能障碍综合征患者病情,合并PT、a PTT及PLT水平异常患者死亡率较高。
