PKP与保守治疗骨质疏松性胸腰椎压缩骨折的临床对比性研究 被引量:8

摘要 目的观察经皮球囊椎体后凸成形术(Percutaneous kyphoplasty,PKP)与保守治疗骨质疏松性胸腰椎压缩性骨折的临床疗效及并发症。方法 2012年8月至2014年2月本院收治的骨质疏松性胸腰椎骨折患者67例,采用随机数字表法分为两组,采用PKP治疗30例(手术组),保守治疗37例(对照保守组)。对比两组病例治疗前后症状改善程度,比较椎体高度恢复率、后凸矫正率,发生邻椎骨折情况、疼痛视觉评分(VAS)、Dallas疼痛问卷(Dallas pain questionnaire,DPQ)评估、Barthel指数、椎体高度测量及Cobb角测量。结果 PKP治疗(手术组)的卧床天数、住院天数较保守组缩短,止痛效果显著、VAS评分、Dallas疼痛问卷(dallas pain questionnaire,DPQ)评估、Barthel指数、椎体高度恢复、Cobb角下降程度均优于保守治疗组,2组比较P<0.05,差异具有统计学意义。结论 PKP治疗骨质疏松性胸腰椎压缩性骨折,患者可迅速缓解疼痛,恢复病椎高度和纠正Cobb角;但有术后再骨折及发生邻椎骨折的风险,且远期疗效有待进一步研究。保守治疗可以使疼痛症状缓解,治疗费用较少,改善患者生活质量不显著,恢复椎体的高度及后凸角不明显,且治疗周期较长,并发症发生率高。
