分段外剥内扎联合断桥整形缝合术治疗环状混合痔的临床观察

Clinical Observation for the Treatment of Circumferencial Mixed Hemorrhoids with the Combination of Segmented External Peeling and Internal Binding with Bridge Cutoff Plastic Surgical suture

目的观察分段外剥内扎联合断桥整形缝合术治疗环状混合痔的临床疗效。方法将120例环状混合痔患者随机分为两组,治疗组采用分段外剥内扎联合断桥整形缝合术,对照组采用传统外拨内扎术。观察两组临床疗效、创面愈合时间,疼痛指数,术后并发症情况。随访半年患者肛门功能、满意度及复发情况。结果 1两组手术有效率均为100%,但治疗组治愈率为96.67%,观察组治愈率为83.33%,治疗组治愈率优于对照组,P<0.05;2治疗组术后疼痛指数、创面愈合时间、并发症发生率,复发率、肛门不全失禁率、满意度优于对照组,P<0.05。结论分段外剥内扎联合断桥整形缝合术治疗环状混合痔疗效可靠,治愈率高,并发症少,创面愈合时间短,复发率低,不影响肛门功能,兼顾了肛门美观,是一种较理想的手术方式,值得推广应用。

Objective To observe the clinical curing effect of treatment of circumferencial mixed hemorrhoids with the combination of segmented external peeling and internal binding with bridge cutoff plastic surgical suture.Methods One hundred and twenty patients with mixed hemorrhoids were divided into two groups randomly.The treatment group received the treatment with the combination of segmented external peeling and internal binding with bridge cutoff plastic surgical suture,while the control group receives the treatment with traditional external peeling and internal binding.The clinical curing effect,wound healing time,pain index and postoperative complications of the two groups were observed.The anal functions,satisfaction and relapseand were followed up.Results The effectiveness of both kinds of surgeries were100%.However,the curing effect of the treatment group is 96.67%,while that of the control group is83.33%.The curing effect of the treatment group was better than that of the control group and P〈0.05.In terms of postoperative pain index,wound healing time,incidence of complications,relapse rate and incontinence rate due to anal insufficiency,the treatment group were better than those in the control group（P〈0.05）.Conclusion The treatment of circumferencial mixed hemorrhoids with the combination of segmented external peeling and internal binding with bridge cutoff plastic surgical suture is reliably effective with high curing effect,few complications,short time of wound healing and low relapse rate.It does not affect the anal functions,maintains the appearance of the anus and effectively solves difficulties in clinical treatment.Thus,it is an ideal way of surgery that should be widely applied.
