去骨瓣减压术在高血压脑出血手术治疗中的应用

Application of decompressive craniectomy in surgical treatment for patients with hypertensive intracerebral hemorrhage

目的探讨去骨瓣减压术治疗高血压脑出血的临床疗效与安全性。方法收集2008-01—2014-06我院行血肿清除术联合去骨瓣减压术治疗的高血压脑出血患者55例作为观察组,同期行单纯血肿清除术治疗者43例作为对照组,比较2组临床疗效。结果观察组优良率为65.5%,显著高于对照组的34.9%;治疗后2组NIHSS评分均显著降低,且观察组显著低于对照组(P<0.05);观察组ADL分级显著低于对照组(P<0.05)。结论去骨瓣减压术治疗高血压脑出血有利于改善患者的神经功能缺损症状与日常生活活动能力,疗效显著,值得推广应用。

Objective To investigate the clinical efficacy and safety of decompressive craniectomy for patients with hyper‐tensive intracerebral hemorrhage. Methods 55 cases of patients with hypertensive intracerebral hemorrhage receiving hemato‐mas cleaning operation combined with decompressive craniectomy treatment in our hospital between January 2008 and June 2014 were collected as the observation group ,43 cases of patients with hypertensive intracerebral hemorrhage receiving simple hema‐toma treatment in the same period were collected as the control group. The clinical efficacy of the two groups were com‐pared.Results The excellent rate of the observation group was 65.5% ,significantly higher than 34.9% in the control group（P〈0.05）;After treatment ,the NIHSS scores of the two groups were significantly reduced ,and the observation group was sig‐nificantly lower than the control group （P〈0.05）;the ADL classification of observation group were significantly lower than the control group （P 〈0.05）. Conclusion Decompressive craniectomy for hypertensive intracerebral hemorrhage can improve the symptoms of neurological deficits and activities of daily living ability ,it＇s effect being significant ,and should be widely ap‐plied.
