中医多途径综合疗法治疗肛肠术后尿潴留临床研究

Clinical Study on TCM Comprehensive Therapy in the Treatment of Urinary Retention after Anorectal Operation

目的:观察用中医多途径综合疗法治疗肛肠术后尿潴留的临床效果。方法:选择2014年3月—2014年10月就诊于我院的102例肛肠术后尿潴留患者为研究对象,随机分为对照组和观察组各51例。对照组患者给予膀胱区穴位按摩、水流声诱导及臀部肌肉注射甲硫酸新斯的明治疗;观察组患者在此基础上给予中药汤剂辨证内服、针灸、耳穴贴压、中药烫熨等多途径综合干预治疗为主。治疗后观察患者自行排尿时间、排尿完全通畅时间、膀胱残留尿量,对患者的术后进食时间、排便时间及住院时间进行统计,计算临床治疗的有效率。结果:经治疗后与对照组相比,观察组患者的自行排尿时间、排尿完全通畅时间明显缩短,膀胱残留尿量明显较少,组间比较差异性显著(P<0.05);观察组患者的术后进食时间、排便时间明显早于对照组患者,住院时间明显较短,组间比较差异性显著(P<0.05);对照组有效率为74.5%,观察组有效率为92.2%,对照组有效率明显低于观察组,组间比较差异性显著(P<0.05)。结论:中医多途径综合疗法能有效改善肛肠术后患者的尿潴留症状,利于促进患者排尿,缩短病程,疗效确切,凸显出中医药多途径论治的优势与特色,值得临床深入研究。

Objective： To observe the clinical effect of TCM comprehensive therapy in the treatment of urinary retention after anorectal operation. Method ： 102 cases of urinary retention after anorectal operation in our hospital from 2014 March to 2014 October were selected as the research object, randomly divided into control group and observation group, 51 cases in each group. Patients in control group were treated by aeupoint massage of bladder region, sound of flowing water induction and intramuscular injection of neostigmine methyl sulfate treatment ; based on this patients in the observation group were given comprehensive intervention of traditional Chinese medicine in many ways, include decoction of Chinese medicine syndrome differentiation, acupuncture, auricular plaster, Chinese medicine hot ironing treatment etc. After treatment observed self voiding time, complete patency time, bladder residual urine volume, eating time, defecation time and hospitalization time, calculated the rate of clinical treatment efficiency. Results ： After treatment, the self voiding time, complete patency time were shortened obviously of cases in observation group compared with control group, bladder residual urine volume of cases in observation group was less, the differences between two groups had significant （ P〈0.05 ）; eating time and defecation time of cases in observation group were significantly earlier than that in control group, the hospitalization time was short obviously, the differences between two groups had significant （ P〈0.05 ）; The rate of efficiency of control group was 74.5%, cases in observation group was 92.2%, the differences between two groups had significant （ P〈0.05 ）. Conclusion ： Traditional Chinese medicine multi way of comprehensive therapy can improve the symptom of urinary retention after anorectal operation effectively, promot the micturition function, shorten the course of disease, curative effect is exact, and highlights the advantages and characteristics of traditional Chinese medicine, so it is worthy of further clinical research.
